ORGANIZATION The final responsibility for the Ouality Assurance Program for Part 71 recuirements rests with Wisconsin Industrial Testing, Inc.

Desian and tabrication or radi oacti ve ma t eri al shippino packages shall not be conducted under this Ouality Assurance Program.

The Ouality Assurance Program is imnlemented usano the followina organizataon.

The Radiation Satety Otticer is responsible tor overall admindatration at the prooram, trainino and certification, document control, and auditina, any or all of these duties may be delegated to the Assistant Rad 2 a tion ott icer.

The Radicaraphers are responsible tor handlino. storing, shippina, inspection, test, ope ra t i ng statue and record keepi no.

OUALITY ASSURANCE PHOGRAM The manaoement of Wisconsin Indus t ri a l Testino, Inc. establishes and implements this Quality Assurance Program.

Training for all OA f uncti ons, pri or to engagement in these functions, is required accordi.nq to written procedures with management approval.

The QA Procram will ensure that all defined OC procedures, engi neeri ng procedures and specitic nrovisions at the onckage desion approval are sa t i s tl ed.

The OA Procram will emphasize control at the characteristics at the packace which are critical to satety.

AUDITS 4

Ertablished schedulen of audi ts of the Oualitv Assurance Program wil1 he certormed using written checklists.

Results of audits will be maintained and reported to management.

Audit reports will be evaluated and deficient areas corrected.

The audits will be dependent on the safety nianificance of the activi ty being audited, but each activity l

will be audited at least once per year.

Audit reports will be maintained as part of the quality a su. a r an c e records.

Members of the audit team nhall have no responsibility in the activity being audited.
