This acknowledges receipt of your Report of Proposed Activities in Non-- .

Agreement States (NRC Form 241) (dated November 12,1992) and receipt of

) your $640 reciprocity recognition fee (check dated November 11, 1992) as i required by 10 CFR 170.31, under Category 16 for Reciprocity. This notification and accompanying fee were referenced in your letter dated November 11, 1992, in response to o"r earlier Notice of Violation.

l Your Florida State License No.-2106-1 is valid for use in non-Agreement l States for up to 180 days in a calendar year, of which you have indicated l 4 you have already used 90 days. In calendar year 1993, a new NRC_ Form 241

{ and reciprocity fee will be required for activities where NRC retains

jurisdiction. This notif % on must be made at least three days in advance of beginning licensed activities. '

Your November 11, 1992, letter states that you will file an NRC Form 241 l when required by 10 CFR 150.20(b). You must also notify us concerning any l revisions, by letter, containing the same information as in Sections 1-7 of NRC Form 241. There is no additional fee required.

! You must also comply with our 10 CFR 34 requirements concerning. radiography l equipment, alarming ratemeters and radiographers and assistant

radiographers; NRC rules may differ from Florida rules._ Fe understand that

! you already possess an NRC-approved package Quality Assurance Plan. You must also register as a secondary user of a package for which a specific license, certificate of- compliance, or other approval has been issued under 10 CFR 71, and must comply with other NRC requirements referenced therein.
