REQUALIFICATION RETAKE EXAMINATION AND INSPECTION
REPORT 50-245/92-27
This refers to the inspection conducted and requalification retake examinations administered
by Mr. D. Florek and others from November 3 - 6,1992, at your Millstone Unit 1 Nuclear
Power Station. The examination was conducted in accordance with NUREG-1021, " Operator
Licensing Examiner Standards," Rev. 6. The inspection independently evaluated stress as a
causal factor fot the crew failure during the NRC administered examination the week of
September 21,1992. Your staff actions to satisfy item 4 of Confirmatory Action Letter 1-
92-014 were also reviewed. The actions are important to safety because they assure your
Licensed Operator Requalification Program is returning safe operators to shift duties.
On November 6,1992, at the conclusion of the inspection, findings related to the preparation
and administration of the requalification retake examinations and inspection findings were
discussed with Mr. H. Haynes and other members of your staff identified in the enclosed
report.

The three major conclusions of the report follow. The first conclusion is that all three
operators and the senior operator passed the requalification retake examination. Generic
weaknesses in the remediation program were also identified as feedback to the training
department. Second, the NRC _ administered examination the week of September 21,1992
provided valid insights of weaknesses in the failed crew's knowledge and teamwork skills.
Stress was not a singular cause of the crew's failure but was a causal factor that exacerbated

the performance problems caused by these weaknesses. The third conclusion is that your
staff has complied with item 4 of Confirmatory Action Letter 1-92-014.
In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of this letter and
the enclosures will be placed in the NRC Public Document Room.
