Thio refers to the routine safety inspection conducted by

R. h

Lerch, K. Salehi, K. Shembarger, and W. Gleaves of this

office on October 5 - 29, 1992.

The inspection included a review

'

of activities authorized for your Zion Station Units 1

and 2.

At the conclusion of the inspection, the findings were

discussed with those members of your staff identified in the

enclosed report.

The inspection focused on engineering and technical support for

station activities.

The specific areas examined are identified

in the report. Within these areas, the inspection consisted of a

selective examination of procedures and representative records,

observations, and interviews with personnel.

Management actions to increase the involvement of engineering

organizations in plant activities was effective.

An example is

the corporate engineering effort for snubber reduction.

Engineering products were typically thorough and correct.

Our

.

inspection, however, found some areas where your performance was

lacking.

In these areas, certain of your activities, appeared to

be in violation of NRC requirements as specified in the enclosed

Notice of Violation (Notice).

The first violation is of concern

'

bccause the engineering evaluations for a temporary alteration to

the safety injection system failed to evaluate the potential for

,

leakage to create a significant water hammer.

An evaluation by

the corporate engineering staff also failed to recognize this

issue.

Other temporary alterations reviewed had evaluation

weaknesses to a lessor extent.

The second violation involved an

interlock between emergency core cooling system valves not being

'

tested after maintenance was performed.

A maintenance activity

unintentionally disabled the interlock.

Upon discovery by the

licensee, the deviation report (DVR) failed to identify

inadequate post-maintenance testing as a contributing cause.

instructions specified in the enclosed Notice when preparing your

response.

In your response, you should document the specific

actions taken and any additional actions you plan to provent

recurrence.

After reviewing your response to this Notico,

including your proposed corrective actions, and the results of

future inspections, the NRC will determine whether further NRC

enforcement action is necessary to ensure compliance with NRC

regulatory requirements.

In accordance with 10 CFR 2.790 of the Commission's regulations,

a copy of this letter, the enclosed inspection report, and your

response (s) to this letter will be placed in the NRC Public

Document Room.

The responses directed by this letter is not subject to the

clearance proceduros of the Office of Management and Budget as

required by the Paperwork Reduction Act of 1980, PL 96-511.

We will gladly discuss any questions you have concerning this

inspection.
