Dear Mr. Smith:

SUBJECT: COMBINED INSPECTION REPORT NOS. 50-352/92-27 AND 50-353/92-27

This letter transmits the results of the resident safety inspection by Messrs. T. Kenny and L.

Scholl, from October 4 - November 14, 1992, at the Limerick Generating Station, L,imerick,

Pennsylvania. The inspection focused on the observation of activities important to ensuring

the public health and safety. Reviews of documents and interviews of plant personnel were

also performed. The results have been discussed with Mr. J. Doering of your staff.

You will note there is a violation for failure to take adequate corrective action regarding

procedural compliance. The NRC has identified several procedural violations of the Limerick

Station in the past two years. There is evidence that corrective actions have been taken by

your staff, however, it is not evident that these corrections have been fully effective. We

note a decline in the occurrence of procedural violations. However, we have observed a

number of procedural non-compliances involving personnel not thoroughly familiar with the

evolution. Maintenance personnel appear to be using procedures without fully understanding

them or the procedures themselves do not fully address the evolution. We would like to

discuss procedural non<ompliance, and problems with inadequate corrective actions at a

Management Meeting in the near future. We will contact you to arrange this meeting.

A second violation involving lack of corrective action has also been issued regarding the

operability determination of check valves in the Reactor Core Isolation Cooling (RCIC)

system. In light of the fact that a 10 CFR part 21 had been issued on this particular class of

check valves, more aggressive corrective actions would have been appropriate.

You are required to respond to this letter and should follow the instructions specified in the

enclosed Notices when preparing your response. In your response, you should document the

specific actions taken and any additional actions you plan to prevent recurrence.
