Contrary to Section 6.5 of the Technical Specifications, Administrative Control Directive 4 ACD 3.6 was not adhered to when on February 2, 1978, work activities authorized by Work Request Authorization 78-147 vere extended beyond the scope of the approved VRA.

This infraction has been discussed with the personnel involved and was discussed during the annual Q.A. training session for plant personnel. The philosophy of the WRA and i.he requirements of the directive were emphasized.

We believe that this action will minimize the potential for similar occur-rences in the future.
