- SEQUENTIAL NUMBER 001 Subsequent maintenance activities and inspections were performed on 1J EOG number 1 lower piston and the upper piston pin bushings for number 2, number 3, and number 8 with no indication of abnormal wear or degradation.

- 1. Engineering will review and modify the elemental analysis wear metal criteria for the EDG lubricating oil to establish a more conservative criterion than vendor recommended thresholds.
- 2. Engineering will present the findings of this event to the Fairbanks Morse Diesel Owners Group for consideration and development of a more conservative elemental wear metal analysis for the Fairbanks Morse Opposed Piston diesel engines in nuclear service.
- 3. Organizational and programmatic analysis review will occur as this event has identified an over reliance on vendor supplied monitoring criteria, specifically related to oil analysis.
s,o Actions to prevent Recurrence Engineering will review and modify elemental analysis wear metal criteria for the EOG lubricating oil establishing a more conservative criterion than vendor recommended thresholds. Lessons learned from this event identified by over reliance on vendor supplied monitoring criteria will be included in leadership training.
