Fitn::co for Duty Progrcm Six Month Performance Report FITNESS FOR DUTY PROGRAM
SUMMARY
During this reporting period, approximately 24.7 percent of the population have been randomly tested. There was one positive test resulting from random selections. The rate of positives from random testing is 0.4 percent.
The HEIDI software used for Fitness for Duty was replaced with a software program developed by Entergy. This new software reduced the cycle time for pulling random lists and entering results and increased department efficiency.
POSITIVE RESULTS:
Of the substances tested for, marijuana is the drug of preference accounting for 57.1 percent of the positive results and cocaine is second yielding 28.6 percent of positives. This is consistent with what has been detected at Waterford 3 in previous submittals. Entergy uses a more stringent cut-off for marijuana. The lower cut-off accounted for half of the marijuana positives.
MANAGEMENT ACTIONS:
Access was denied for the employees that tested positive during pre-access and random testing. Management was notified and the individuals' files were annotated of the positive results and management's actions.
There were two empioyees participating in follow-up programs. One employee tested positive during a follow-up test. This employee's access has been denied and is no longer with the Company. Four follow-up tests were administered collectively to these employees.
EVENTS REPORTED TO THE NUCLEAR REGULATORY COMMISSION (NRC):
During this reporting period, there were no violations of the fitness for duty program that were reportable.

PROGRAM WEAKNESSES:
5 Two incidents occurred during this reporting period that were documented as program weaknesses.
- 1) One split sample from a confirmed positive that was stored on site was i
inadvertently discarded prior to the one year retention period. The confirmatory laboratory was contacted to ensure a partial split was still in storage at their facility.
The procedure for destruction of urine samples was revised to prevent this event from reoccurring.
- 2) One individual that was notified to report for random testing forgi to report. This was considered a refusal to test and the individual's access was removed for-cause.
The individual came in for for-cause testing and access was reinstated upon receipt of nogative results of the test. The process for the tracking of scheduled random participants was revised.
