Vol. 1, issued July 1992, summarizes events that occurred and were reported from pre-NRC through December 31, 1989. Vol. 2, Rev. 4, revised July 1996, summarizes events that occurred from January 1, 1990, through December 31, 1995. Events which involve only source material, byproduct material, or natural uranium which are not safeguarded under NRC regulations are not included, except for a few events included in the Miscellaneous category because of public interest. Information on events involving nonsafeguarded material is available in NUREG-1272 published annually by the Nuclear Regulatory Commission. NUREG-0525, VOL. 2, REV. 4 JULY 1996 l INTRODUCTION The Safeguards Summary Event List (SSEL) provides brief summaries of several hundred safeguards-related events involving nuclear materials or facilities regulated by the U.S. Nuclear Regulatory Commission (NRC). The SSEL is comprised of two parts. Vol. 1, issued July 1992, summarizes events that occurred and were reported from pre-NRC throu@ December 31, 1989. Vol. 2, Rev. 4, issued July 1996, summarizes events that occurred January 1,1990, through December 31, 1995. Because of public interest, the Miscellaneous catego n includes a few events which involve either byproduct material, source material, or natural uranium which are exempt from safeguards requirements. As the list is intended to provide a broad perspective on the nature of safepards incidents in the licensed nuclear industry, both unusual and routine events are described. Events have been placed in the several categories described below. Each listing is included in a single category unless it is transportation-related, in which case it is cross-referenced in the Transportation category. I. Bomb-Related: Events concerned with explosives or incendiary devices. Section A: Events in which a bomb or explosive material was located or an explosion occurred at a licensed facility. Section B: All other bomb-related events (e.g., hoax bomb threats). II. Intrusion: Incidents of attempted or actual penetration of a l facility's barriers or safeguards systems. l/,s\\ stolen, alleged to be stolen, discovered missing, or found. III. Missing or Allecedly Stolen: Events in which safeguarded material was i '\\ IV. Transoortation-Related: Events typically include incidents where safeguarded material was misrouted or involved in an accident. Material reported missing or stolen during transport, for example, would be included in Category III and cross-referenced in the Transportation category. V. Tamoerine / Vandalism : Incidents of destruction or attempted destruction of property, parts and equipment which do not directly cause a radioactive release. VI. Arson: Intentional acts involving incendiary materials resulting in damage to property, equipment or other assets.

VII, Firearms-Related:

Events typically describe the discharge, discovery, or loss of firearms at a licensed facility. l l VIIIA. Radiolozical Sabotace: Deliberate act directed against a safeguarded l activity which could endanger the public health and safety by exposure to radiation. VIIIB. Nonradiolocical Sabotare: Events characterized by the FBI as sabotage but which could not endanger the public health and safety by exposure to radiation. IX. Alcohol and Druggi These events have been dropped from this document and are now contained in NUREG/CR-5758. X. Each incident listing includes the appropriate date and location and a brief description of the event. At 9:11 a.m., the Waverly, Ohio, Police Department reported to the Portsmouth Gaseous Diffusion Plant (GDP) that they had received an anonymous telephone call stating that there was a bomb at Martin Marietta, the management contractor for the USEC's GDP at Piketon, Ohio. Action: Searches were conducted.

At 11:57 p.m., the Piketon, Ohio, Police Department received a telephonic bomb threat from an individual who stated that there was a bomb at Martin Marietta. Action: A search was conducted. Resolution: The search results confirmed that the threat was unsubstantiated. Source: Licensee 1B-95-05 Date: 05/09/95 Site: Massachusetts Institute'of Technology' ~ Cambridge, MA-Plant Status: Operational Fuel present at site. Event: Bomb threat. Hoax. At 2:05 p.m., the secretary to the Director of the Massachusetts Institute of Technology Nuclear Reactor Laboratory received an anonpous telephonic bomb threat from a male on an outside line. The caller stated that a bomb had been planted next to the reactor. Action: The reactor was shut down, special nuclear material was secured, and the reactor facility and adjacent administrative building were evacuated. Campus police, Cambridge police, and fire and bomb disposal personnel responded, and a search was conducted. Resolution: No explosive device was detected, and the buildings were reentered at 3:15 p.m. Source: Licensee IB-95-06 Date: 05/12/95 Site: SW; Virginia Electric Power Company Surry County, VA Plant Status: Unit 1, @ erational Unit 2, Shutdown Fuel present at site. Event: Bomb threat. Hoax. At 4:16 p.m., the 911 operator in Lexington, Kentucky, received a bomb threat, directed at the local area in general, from an individual with a child-like voice. The operator interpreted the call as a threat to Surry and Smithfield, Virginia. Action: As a precautionary measure, a search of the station was conducted.

Enhancement of the messagen revealed the following words: " bomb", "in nuclear reactor building", "6 million", " hurry up", and "now". i Action: The facility was secured, the confinement building was evacuated of nonessential personnel, and the complex was searched. 1 Resolution: No explosive device or other unusual item was found. At 7:30 a.m., a third garbled, unintelligible voice mail message was received on the office telephone of the NIST Chief of Facility Services. Action: As e precautionary measure, the building, the roof of the building, and outside grounds were searched. Resolution: No explosive device or other unusual item was found. Commonwealth Edison Company : Will County, IL: Plant Status: Unit 1, Shutdown Unit 2, Operational Fuel present at site. Event: Bomb threat. Hoax. At 7:20 a.m., the Braidwood Police Department received a telephone call from an anonymous individual who stated that two bombs would detonate at Braidwood that day; one at 8:30 a.m. and one at 9:30 a.m. The information was communicated to the Braidwood Station and to the Will County Sheriff's Department who responded to the site.

II-l INTRUSION NUREC-0525, VOL. 2, REV. 4 JULY 1996 'N 'II-93-01L Date: 02/07/93 Site: (Three Mile Island ' CPU Nuclear Corporation Dauphin County, PA Plant Status: Unit 1, Operational Fuel present at site. Event: At 6:53 a.m., an unauthorized vehicle drove through the open, but manned, north gate in the owner-controlled area, drove through the parking lot to the protected area, crashed throu a gate in the protected area fence, smashed into the turbine b 11 ding roll-up i doors, and eventually came to a halt approximately 63 feet inside l the turbine building. The driver exited the vehic'.e and fled; his whereabouts were not immediately known. l Action: Immediately after the intruder passed through the "cith gate, security notified the central alarm station who, in curn, notified I the control room. Designated vital areas were secured with armed personnel and vital access restricted via key cards. A security emergency was declared at 6:55 a.m., a site area emergency was declared at 7:05 a.m., and appropriate notifications were made. At 7:20 a.m., the Pennsylvania State Police arrived on site. At 8:38 a.m., the first search and clear operation began in the turbine building with negative results. At 9 a.m., the U.S. Army 56th Explosive Ordinance Team initiated a check of the vehicle for explosives with negative results. At 10:57 a.m., during the conduct of the second turbine building search and clear operation, the intruder was found underneath circulating water p pin on the h bottom floor of the turbine building and apprehended y s te security. He was released to the Pennsylvania State Police. d Resolution: Augmented security measures have been im lemented by the licensee. In addition, because of this event and t e February 1993 bombing of the World Trade Center, the Nuclear Regulato Commission decided to revise the design basis threat for radiolo ical sabotage, 10 CFR 73.1, to include land-based vehicles against which nuclear power plant licensees would develop protection systems. Event: A female plant visitor activated the intrusion alarms when she inadvertently entered a protected area by opening and passing through two doors which a guard had failed to properly secure. Action: Security personnel were immediately dispatched to the site, but the individual had exited the building and driven away in her automobile, Resolution: Security contacted the intruder who stated that she had gotten lost and had gone into the building, opened the doors to find herself in a closed vestibule with the inner doors locked, and then left. (N There was no malicious intent. The security officer who improperly )( ) secured the doors was counseled and subjected to disciplinary action. Source: Licensee

Event: A loaded weapon was identified in a cleaning contractor's hand-carried purse during entry search. Action: The contraband was confiscated, the individual was denied access, \\ and the contractor's employer was notified. Resolution: The licensee determined that the individual, who had a valid permit to carry the weapon, had inadvertently forgotten to remove the gun prior to reporting for work. The weapon was in a ^ holster, concealed under a blanket, in the sleeping compartment of the vehicle cab. The ammunition was hidden in a western (cowboy) style boot in the vehicle's cab. Action: The weapon and ammunition were secured, and assistance from the Wake County Sheriff's Department was requested. Resolution: The vehicle and its cargo were removed from the vehicle sallyport and parked in the licensee's south parking lot. Action: The licensee's security force responded to the landing after plant employees reported the-incident. Upon notification, the State police and the sheriff's office responded to the site and determined that the airplane carried no contraband. After a roughly 151 percent increase in events reported between 1978 and 1979 (the result of a change in reporting requirements, an increase in events reported following the TMI March 1979 occurrence, and contentious licensing hearings), totals reported by year declined somewhat and remained relatively stable until 1986 (96*) which experienced a 43 percent increase over 1985 (67). Events reported remained stable in 1987, increased in 1988 (137) and 1989 (148), declined by 30 percent in 1990 (103) from 1989, but increased by 48 percent in 1991 (152) from 1990, due to an increase in bomb threats and miscellaneous threats reported in the first few months of 1991 during the Persian Gulf crises, as well as an increase in events involving safeguards information and contraband. During 1992, the 94 events reported reflected a decline of 38 percent from 1991, primarily because of a drop in bomb-threats and miscellaneous events reported. In 1993, the 116 safeguards events reported represented an increase of 23 percent from 1992 as a result of an increase in firearms events reported. In 1994, there were 118 events reported, reflecting an increase of 1.7 percent over 1993. The rise was attributable to the increase in the miscellaneous category; all other O categories of reported events declined. In 1995, there were 58 events reported, a 51 percent drop over 1994, largely in the firearms and miscellaneous categories. With a few exceptions, only significant events have been included during this period, reflecting discontinuance of the safeguards event quarterly log reporting to NRC.

Although reactor safeguards events declined 29 percent in 1990 (100) from 1989 (142), there was an increase of 50 percent in 1991 (150) over 1990 for the reasons stated in Figure 1. During 1992, there were 94 reactor safeguards events reported, reflecting a decline of 37 percent from 1991, primarily in bomb threats and miscellaneous events reported. Reactor safeguards events reported in 1993 rose to 116, an increase of 23 percent over 1992, attributable to the increase in firearms events reported. In 1994, there were 118 events reported. 1 Figure 4 shows the trend in nonhomb-related safeguards events. After a marked increase in nonbomb-related safegurds events of 67 percent in 1988 (100) over 1987 (60), there was a moderate increase of 23 percent in 1989 (123) over 1988, and a decline of 25 percent in 1990 (92) from 1989. However, 1991 (120) increased by 30 percent over 1990, for some of the reasons stated in Figure 1. During 1992, there were 82 nonbomb-related safeguards events reported, reflecting a decline of 32 percent from 1991, primarily in the reported miscellaneous events (safeguards informatic,, general threats, contraband, etc.). Bomb threats, which declined considerably after reaching a peak of 94 in 1979, reached a low level in 1990 (11), a 56 percent drop from 1989 (25). Of the 11 events in 1990, 4 occurred at a facility granted a full power license in March 1990. However, there was a 191 percent increase in 1991 (32) over 1990; 16 (50 percent) of the 32 were received during January and February and may be attributed to the crisis in the Persian Gulf. During 1992, there were 12 bomb-related events versus 32 for 1991, a decrease of 63 percent. Three power reactor facilities received 2 threats each through the 3rd quarter accounting for 50 percent of the threats received, and 1 threat was directed at a nonpower reactor facility (. During 1993, the number of bomb-related events reported remained stable 12), all at power reactor facilities. Bomb-related events reported reached an all-time low in 1994 (9), a 25 percent drop from 1993, and one facility accounted for 3 of the reported events. During 1995, there were 12 bomb-related events reported, an increase of 33% over 1994. Of the 12 events, 7 (58) percent occurred at power reactor facilities; 3 5 (25) percent at nonpower reactors; and 2 (17) percent at fuel facilities.

After rising steadily, increasing by 100 percent in 1988 (56) vs. 1997 (28), by 7 percent in 1989 (60) over 1988 and 20 percent in 1990 (72) from 1989, there was a decline of 19 preent in 1991 (58) and a further decline of 10 percent in 1992 (52) in firearms reported events. It should be noted that from 1987 to date, there has been an increase in events reported involving ammunition detection (no weapon involved). Of the 72 firearms-related events reported for 1990, 43 (60 percent) iuvolved weapons and 29 (40 percent) ammunition only; in 1991, 31 (53 percent) involved weapons and 27 (47 percent) ammunition only; in 1992, 22 (42 percent) involved weapons and 30 (58 percent) involved ammunition only; and in 1993, 35 (49 percent) involved weapons and 37 events (51 percent) involved amerunition only. In 1994, firearms events reported (63) declined by nearly 13 percent from the previous year (72). Thirty-five (56 percent) involved weapons and 28 (44 percent) involved ammunition only. During 1995, firearms reported (25) declined by (60) percent from 1994, due to the reasons stated in Figure 1. Of the firearm events reported, 16 (64) percent involved weapons and 9 (36) percent involved ammunition.

This category includes not only safeguard events which do not proper 1 fit into any of the categories defined in the document, but also covers events which involve persons or materials not subject to NRC safeguards regulations, but which are of interest to the Cossaission, the industry, and the public. There was a rise in the miscellaneous events reported of 129 percent in 1988 (32) over 1987 (14), a rise of 47 percent in 1989 (47) over 1988, a decline of 64 ercent in 1990 (17) from 1989, but a marked increase of 253 percent in 1991 (60. Once again, significant numbers of events occurred in January and February of 1991 and may be attributed to the Persian Gulf crises. The 1991 high of 60 events was followed by a decline of 55 percent in 1992 (27). Miscell:neous events reported remained stable during 1993 (27). In 1994, the miscellaneous events reported (43) increased 59 percent over 1993. Twenty eventa (46 percent) related to safeguards information (151993); 5 (12 percent) to contraband other than firearms (6 1993); 3 (7 percent) to general threats (4 1993); and 15 (35 percent) to other types. Note that the miscellaneous category of events reported is the only category of events which increased in 1994; all other categories declinad. ( During 1995, the 20 miscellaneous events reported reflected a decline of 53% from the prior year, 6ue to the reasons stated in Figure 1. No events related to safeguards informacion were reported.
