NRC INSPECTION REPORT #85-05 NOTICE OF VIOLATION HOPE CREEK GENERATING STATION Your letter dated March 21, 1985, transmitted the above referenced Inspection Report which contained a Notice of Violation citing one (1) item of noncompliance concerning the lack of formalized controls on piping system temperature during startup flushing operations. The following response is provided in accordance with the Notice of Violation.

As of February 14, 1985, during RHR system flushing, control and monitoring of piping system temperature, a factor affecting quality due to a 1200F limit with system spring-can type hangers pinned, was not prescribed by documented instructions, proce-dures, and drawings. Furthermore, this lack of formal control and monitoring of temperature was typical of all flushes of piping systems whose hangers were pinned.

Corrective Steps Taken and Results Achieved

.The Public Service Startup Group has not been negligent in its respcc.libility to monitor and maintain system temperatures below th, 1200F limitation in accordance with specification P-410 (Q). , " Installation, Inspection, and Documentation of Pipe Support in Nuclear Service". Our investigation revealed that twelve (12) Q systems had undergone system flushing with 8504290346 850419 PDR -ADOCK 05000354 PDR G

0 Dr. T. E. Murley 2 4/19/85 pinned hangers prior to the Inspector's finding. A detailed listing of these systems and the associated monitoring measures taken was provided to the Senior Resident Inspector for his review. To date, the 1200F temperature limit has not been exceeded on any system flushed with the hanger pins installed.

Corrective Steps Taken to Preclude Recurrence Startup General Test Procedure, GTP-1, " General Flushing and Cleaning Procedure", has been revised to incorporate a piping temperature monitoring program. Implementing instructions are contained in Startup Instruction SI-41, " Flushing Constraints".

Monitoring of piping system temperature will be effected through the use of temperature monitoring strips to be affixed to equipment / piping undergoing flush.

System flushes which are expected to exceed the 120 F limit will be flagged to the attention of Bechtel Resident Engineering for evaluation and provision of additional supports, as required.

The Date of Full Compliance The date of full compliance was March 28, 1985.
