In Reference A the Supply System informed your office of a potentially reportable deficiency 'under 10CFR50.55(e).
Attachment' A provides a_ statement 'of the identifie'd condition and a brief i
description of our planned actions to correct the identified deficiency. Based on the -current construction status at WNP-1/4, the Supply System will not be able to issue a final report at this time.
An update will be provided. at construction

Emergency Diesel Generator Flexible Coupling Drive Hubs Description of Deficiency The Supply System purchased Emergency Diesel Generators, two per plant, from Transamerica Delaval.
Transamerica Delaval has reported under the requirements of 10CFR. Part 21, a deficiency associated with the flexible coupling hubs in the Overspeed Governor and Fuel Oil Booster Pump drives.
There are two affected coupling units per engine..
The hubs are manufactured by Lovejoy Coupling and installed by Transamerica Delaval.
Although the design has been used for many years without similar problems, the drive hubs on a non-nuclear commercial engine were found loose on the shafts.
These hubs are normally installed on the shaft using a key, a setscrew over the key, and a pin.
Analysis of Safety Implication The 0verspeed Governor and Fuel Oil Booster Pump are run off the same drive shaft.
The. Overspeed Governor inboard coupling connects to the engine drive shaft and the outboard coupling connects to the Fuel Oil Booster Pump.
If the Overspeed Governor stops turning,an alarm occurs but the engine will continue to operate. However, if either coupling fails, the Fuel Oil Booster Pump will not provide the necessary oil pressure to maintain engine operation.
Although a DC powered auxiliary fuel oil pump will start on low fuel oil pressure, this pump is not qualified for safety related service and credit cannot be taken for this auxiliary pump.
Cause of Deficiency This deficiency was apparently caused by Delaval's failure to order the coupling best suited for the specific application.
Coupling manufacturers supply the couplings with various types of " locking" devices.
To use this coupling for this specific application Delaval should have specified additional installation requirements to ensure the setscrew will not become loose or fall out.
Corrective Action i
In the 10CFR Part 21 report (1/9/84) Delaval indicated that the necessary-corrective actions as described in the Part 21 report would be issued to each applicable site per Service Information Memo #363. The Supply System has received this information and plans -to implement as written.
Briefly, the two couplings will be inspected and reinstalled as follows;
- 1) Check coupling hub fit by renoving pin and setscrew. If tight reinstall pin and setscrew using Locktite per Manufacturer's recommendation.
2)
If hub is loose, the mating surfaces should be inpsected, cleaned, and reinstalled using Locktite per the Manufacturer's recommendation.
The key, set screw, and pin should then be reinstalled as described above.
The field change to implement this inspection and corrective action is currently in the approval process.
Due to the construction status of WNP-1/4, this change will not be implemented until after construction resumes.

Evaluation of other similar components is not warranted at this~ time as this deficiency appears to be ~ an isolated case where the _ vendor failed to provide adequate specifications appropriate for this application.
