Millstone Nuclear Power Station, Unit No. 3 Reporting of Potential Significant Deficiencies in Design and Construction:

Improper Installation of Conduit Seal for Rosemount Pressure Transmitters (SD-67)

In a December 14, 1984 telephone conversation between your Mr. T. Rebelowski and our Mr. R. E. Lefebvre, Northeast Nuclear Energy Company (NNECO) reported a potential significant deficiency in the construction of Millstone Unit No. 3 as required by 10CFR50.55(e).

The potential significant deficiency involves four Rosemount differential pressure transmitters with improper conduit installations. The Rosemount instruction manual requires a qualified thread sealant be applied where the conduit enters the transmitter. This sealant was not used for these transmitters and is necessary to prevent moisture from accumulating into the terminal side of the transmitter housing during accident conditions. Further investigation noted that several other transmitter conduits t

l had been installed in a similar manner.

Our architect-engineer, Stone & Webster Engineering Corporation, has identified j

all Millstone Unit No. 3 instrumentation that requires a conduit entry seal.

l These conduit seals will be reworked by NNECO on a post-turnover basis and the correct seal installed. However, we are still investigating the scope of this problem and evaluating its potential generic implications.
