Millstone Nuclear Power Station, Unit No. 3 Reporting of Potential Significant Deficiencies in Design and Construction:

Westinghouse Process Control Cabinets (SD-59)

In a September 19,1984 telephone conversation between your Mr. T. Rebelowski and our Mr. G. Ballassi, Northeast Nuclear Energy Company (NNECO) reported a potential significant deficiency in the construction of Millstone Unit No. 3 as required by 10CFR50.55(e). The potential significant deficiency involved process control cabinets supplied by Westinghouse (Reference 1). Wiring installed inside these cabinets was not installed in accordance with the applicable Westinghouse field change instructions.

We have determined that this is a significant deficiency for Millstone Unit No. 3.

NNECO has completed a 100% inventory of all wire terminations within the cabinets, as configured. Utilizing NNECO's inventory, Westinghouse identified all wiring discrepancies between the installed wiring and the sof tware (wiring list and drawings). Two types of problems were discovered; software deficiencies and wiring deficiencies. Westinghouse then corrected all software deficiencies and generated Field Deficiency Reports for corrective action by NNECO.

NNECO has implemented and corrected all wiring deficiencies as identified by the Field Deficiency Reports, in addition, each loop within each cabinet will be verified functional for proper app!! cation during start-up testing and it will be verific'd that no system interaction exists between other loops.
