10CFR Part 21 - Reporting of Possible Field Failure of Space Heaters Supplied on General Electric Motors Gentlemen:

Per the requirements of 10CFR Part 21, Bingham Willamette Company hereby advises that the motors supplied with pumps for Bechtel/ Georgia Power Company, Vogtle Plant may have Wellman Company manuf actured space heatere which could cause a possible field failure. Bingham received notification from General Electric Company, Small AC Motor Department, Mr. Ken Kool, Product Service Engineer, San Jose, California, dated 1/18/85 --Attachment I.

We have formally notified Georgia Power Company of this matter and requested an inspection of motors to determine if any Wellman space heaters were supplied (AttachmentII).

A possible field failure exists if the end plug where the leads ex n arc epoxy, eventually the epoxy carbonizes and the lead wires short out.

This same condition may exist on motors supplied with pumps to Bechtel Power for Taiwan Power. Bingham Willamette Company has also notified Bechtel of this matter, although this would not directly fall under the jurisdiction of 10CFR Part 21. Bingham knows of no other pumps supplied with motors to other nuclear power plants with Space heaters from Wellman Company.

Corrective action has been as follows:

(1) Notification to appropriate customers to inspect motors and report any Wellman Space Heaters simultaneously to Bingham Willamette Company and General Electric Company, no later than February 1, 1985.

(2) General Electric-Company to supply replacement heaters.

(3) Bingham Willamette recommends that Georgia Power negotiate any required replacement heaters with Mr. Ken Kool, General Electric Company, San Jose, California to complete the action required no later than February 28, 1985.

Per the requirements of 10CFR Part 21, Bingham Willamette Company hereby advises that the motors supplied with pumps on Georgia Power P.O. #PAV-625 and PAV2-67 may have Wellman Company manufactured space heaters which could cause a possible field failure (reference letter from General Electric Company, Small AC Motor Department, San Jose, California, dated January 18, 1985, Attachment I).

Attachment II lists BWC Sales Order, model/ size of pumps and equipment identification number of the pumps applicable to P.O. #PAV-625 and PAV-67.

Please inspect the attached equipment and simultaneously notify Bingham-Willamette Company and General Electric Company of any Wdllman space heaters with model and serial number of each motor no later than February 1, 1985.

Bingham-Willamette Company recommends that you directly negotiate supply of any required replacement heaters with Mr. Ken Kool, General Electric Company, San Jose, California, Product Service Engineer.

Bingham-Willamette Company will assist to obtain a timely resolution as requested.

We have purchased this component. from two vendors and recently had some field failures with those manufactured by Wellman Company.

In discussing.this with Wellman, we found that they had changed the end plug where the leads exit frcm ceramic to epoxy.

Eventually the epoxy carbonizes and the lead wires short out.

We suggest that you check the heaters in 'your.notors listed above..If you find you have Wellman heaters, advise us the model and serial numbers of the motor and we will supply replacement heaters free of charge.

Please reply by 2/28/84 latest.
