J. M. Farley Nuclear Plant NRC Inspection of January 14 - January 17, 1985 RE:

Report Nusbers 50-348/85-04 50-364/85-04

Dear Mr. Verrelli:

This letter refers to the violation, cited in the subject inspection reports, which states:

"Tne following violation was identified during an inspection conducted on January 14 - 17, 1985. The Severity Level was assigned in accordance with the NRC Enforcement Policy (10 CFR Part 2, Appendix C).

Technical Specification (TS) 6.8.1(b) requires that procedures be established, implemented and maintained covering refueling activities.

Contrary to the above, on January 16, 1985, the licensee was performing a video inspection to determine possible fuel damage during cycle 3 operation in the spent fuel pool with no procedure governing that activity. This violated TS requirement 6.8.1(b).

This is a Severity Level V violation (Supplement I)."

Reason for Violation The above violation was caused by personnel error as well as procedural inadequacy in that the use of underwater video cameras and recorders for fuel inspection was not covered by a procedure.

Corrective Action Taken and Results Achieved None. This activity was completed by the time the deficiency was identified.

Corrective Steps Taken to Avoid Further Violations Fuel inspection procedures for both units have been revised to provide for the use of underwater video cameras and recorders.
