Director of Nuclear Ekensing General Of' ices: 1945 West Parnall Road, Jackson, MI 49201 (517) 788-1636 March 29, 1985 James G Keppler, Administrator Region III US Nuclear Regulatory Commission 799 Roosevelt Road Glen Ellyn, IL 60137 DOCKET 50-255 - LICENSE DPR PALISADES PLANT - RESPONSE TO INSPECTION REPORT 85-02 Two items of noncompliance were identified in Inspection Report 50-255/85-02 dated February 27, 1985. The following is our response to these items:

Noncompliance (50-255/85-02-01)

Technical Specification 3.3.1 states "The reactor shall not be made critical, except for low temperature physics test unless all of the following conditions are met:

All four Safety Injection Tanks are operable and pressurized to at least 200 psig with a tank liquid level of at least 186 inches (55.5%)..."

Technical Specification 3.3.2.a states that one safety injection tank may be inoperable for a period of no more than one hour.

Centrary to the above, two Safety Injection Tanks were inoperable (level less than 1.86 inches) on December 7, 1984 for a period of 2 minutes.

Corrective Actions Taken and Results Achieved Immediate action was taken to restore the Safety Injection Tanks to the required Technical Specification level. Evaluation of this occurrence determined that an out of calibration level indicator had misled operators and h

failed to warn of the impending low level condition. The indicator was subsequently calibrated. Details of this event was reported in Licensee Event Report 84-026.

l Corrective Action to be Taken to Avoid Further Noncompliance An evaluation of the valve leakage will be performed to identify any additional action necessary to limit leakage. The high boric acid content in this system may restrict any reasonable actions that will reduce leakage. The Safety Injection Tank level system has been a continuing concern and additional action will be evaluated to improve the reliability of the indicators.

Date When Full Compliance Will be Achieved Full compliance has been achieved. Additional actions will be completed during the next refueling outage currently scheduled for completion in early 1986.

i Noncompliance (50-255/85-02-02) 10CRF50, Appendix B, Criterior V states in part: " Activities affecting quality shall be prescribed by documented instruction, procedures...and shall be accomplished in accordance with these instructions, procedures..."

Comsumers Power Company Topical Report CPC-2A, " Quality Assurance Program Description for Operational Nuclear Power Plants", is implemented by Nuclear Operations Department Standards (NODS). N0DS-P13, " Housekeeping and Cleanliness Control", Section 5.1, requires procedures to be established for certain areas and verification of housekeeping requirements to be documented.

Palisades Nuclear Plant Administrative Procedure 4.04, " Plant Housekeeping",

outlines specific requirements for documentation of housekeeping tours.

Contrary to the above, documentation of housekeeping tours has not been accomplished as outlined in the above referenced procedures.

Corrective Action Taken and Results Achieved Administrative Procedure 4.04, " Plant Housekeeping", was revised to clarify the requirements for housekeeping tours. The applicable Operations Department personnel have been directed to comply with the requirements concerning housekeeping tours.
