Dear Sir,

It has been brought to our attention by Bonney Forge and the Nuclear Regulatory Commission that Consolidated Pipe & Supply Company has provided materials to the Grand Gulf Nuclear Station which lacked the chemical overcheck required by Paragraph NCA-3867.4 (e) (2) of Section III of the ASME Boiler and Pressure Vessel Code.

'Ihis material was " drop-shipped" direct from Bonney Forge in Carlinville, Illinois to Grand Gulf in fulfilment of Bechtel Power P.O. to Consolidated Pipe,P.O. #9645-F-25894, Rev 1. dated 9/19/78.

followed by water quenching.

During an internal review of quality assurance records it has come to our attention that some material was delivered to you with incomplete documentation.

The purpose of this letter is to advise you of certain ma te ri a l s shipped from the Carlinville plant lacking the chemical overcheck requirement of Para, tJ C A-3 8 6 7. 4 ( e ) ( 2 ).

It should be noted that we a re talking about the chemical overcheck only; not any e s s e n,t i a l production steps used in the ma nu f ac t u re or steps to assure traceability.

All of the subject material is heat traceable through our own plant, the material suppiers' warehouse and back to the mill of nanufacture.

In addition, all suppliers' warehouses and man-ufacturing mills appear on ou'r audited and approved vendors lists throughout the period in question.

The attached list of purchase order numbers, item numbers, heat numbers and fitting descriptions is a complete list of the affected itens based on an in-depth review of our files back to January 1977.
