Dear Dr. Murley:

Millstone Nuclear Power Station, Unit No. 3 Reporting of Potential Significant Deficiencies in Design and Construction:

Main Control Board Lugs (SD-48)

In a November 10,1983 telephone conversation between your Mr. T. Elsasser and our Mr. 3. 3. Festa, Northeast Nuclear Energy Company (NNECO) reported a potential significant deficiency in the construction of Millstone Unit No. 3 in accordance with 10CFR50.55(e). In Reference (3), we informed you that this is a significant deficiency for Millstone Unit No. 3.

The significant deficiency involved inadequate terminations of #14 AWG Tefzel and ITT exane wire to AMP PIDG 53983-1 16-14 HD lugs in the main control board supplied by Reliance Electric Company. In addition, Reliance Electric identified two other areas of concern in their inspection regarding configurations not included in the original seismic test report. The first of these concerned 16-14 AWG lugs used to terminate 20 AWG resistor leads on Cutler-Hammer Type E-30 switches and on Stanwick terminal blocks. The second area regarded 16-14 AWG lugs that were terminated to the conductors using an ETC crimper.

As reported to you in Reference (3), Reliance Electric completed a 100%

inspection of alllugs within their supplied panels. Our architect-engineer, Stone

& Webster Engineering Corporation, also completed all of the repair work for the improperly crimped lugs in the pre-turned-over panels. NNECO has completed most of the repair work for the lugs in the post-turned-over panels and intends to complete the remaining work by the end of April,1985.

, Regarding; the other.two areas of concern, we informed you that Reliance Electric completed the seismic testing of these two configurations in accordance with the seismic portion of IEEE-323.. Since then, they have finalized their seismic test report. This report does not note any significant anomolies with the lugs'.

In addition, the mild environmental qualification plan (Report Number

SQP-99X2087) was revised accordingly.

We consider this to be our final report for SD-48. Based on a conversation with

,Mr. T., Rebelowski, this report is 'being provided by March 25 rather than February 27, 1985.. We trust that the above information satisfactorily responds
