Millstone Nuclear Power Station, Unit No. 3 Reporting of Potential Significant Deficiencies in Design and Construction:
Nonconforming Terminations in CVI Corporation Equipment (SD-75)
In a February 13, 1985 telephone conversation between your Mr. T. Rebelowski and our Mr. R. K. McCarthy, Northeast Nuclear Energy Company (NNECO) reported a potential significant deficiency in the construction of Millstone Unit No. 3 as required by 10CFR50.55(e).
The potential significant deficiency involves improper terminations found in equipment supplied by CVI Corporation. Termination deficiencies include crimps which were unacceptable per vendor's instructions, terminations crimped with the wrong tools, multiple conductors crimped in one lug, and incorrect lug size used.
l The subject terminations appear in the following safety-related equipment at Millstone Unit No. 3:

HVR*PNLFLT 1 A, IB - Auxiliary Building filtration units.
o HVR*PNLFLT 2A,2B - Fuel Building filtration units.
o HVR*PNLFLT 3A, 3B - Supplementary Leak Collection and Release System filtration units.

o Our. architect-engineer, Stone & Webster Engineering Corporation, is in the process. of developing an inspection and. repair program to correct this deficiency. We will provide you with an update on this matter by June 12,1985.
