~ Enclosed is a review detailing the results of the audit performed on the Environmental Qualification Program documentation at Diablo Canyon on December 19 and 20, 1983.
It was concluded that the audited documentation showed adequate technical basis for the claimed qualification, demonstrated that Pacific Gas and Electric (PG&E) had maintained those files properly, and that PG&E had resolved commitments made during audits previously performed by EQB.

VERIFICATION OF THE ENVIRONMENTAL QUALIFICATION DOCUMENTATION AT DIABLO CANYON
1.0 INTRODUCTION
On December 19 and 20, 1983, a team composed of a representative from the Idaho National Engineering Laboratory (INEL) and members of the Nuclear Regulatory Commission (NRC) staff, audited the Environmental Qualification files for Diablo Canyon. The focus of the audit centered on technical adequacy, evidence of proper file maintenance, and resolution of prior commitments.
2.0 EVALUATION Five files were audited. A brief summation of each file follows:
HH-2 ASCO SOLEN 0ID VALVES This file, one of seven new files that resulted from a high energy line break (HELB) reanalysis, utilized the Isomedix Test Report AQS 21678/TR Revision A, July 1979, as the basis for qualification. Test profiles

enveloped expected environmental conditions, and aging calculations using Arrhenius methodology were performed correctly.
It was concluded that the equipment covered by this file is environmentally qualified, and that

the file was actively maintained.
EH-3 RAYCHEM FLAMETROL CABLE These cables are located in areas that have their harsh environments changed by the HELB reanalysis. Work in progress on this file showed that the equipment was environmentally qualified for the new environment, and that all calculations and analysis were correctly performed.
lH-16 LIMITOROUE SMB SERIES VALVE OPERATORS These valve operators, previously qualified, are located in areas that are affected by the HELB reanalysis. Occumentation in progress, based on the Limitorque Test Report B0003, and Limitorque Report B-0027, showed that the valve operators are_ qualified for the new environment.
An overview of the new technical evaluation, evidenced by engineering calculation M 486, showed adequate technical quality.
1H-21 TEC ACOUSTIC MONITOR This file, which was stated in the September, 1981, Equipment Qualification Submittal to be awaiting test results, was reviewed to determine proper file maintenance, and to verify completion of cccmit-ments made during the Environmental Qualification audits conducted in the past. The review showed inclusion of TEC Test Report 517 TR-03, 7
Revision-2, December, 1981, analysis performed, and file completion on June 28, 1982. The file was complete, the contents technically adequate, and evidence of proper file maintenance was found.
lH-24 BARTON PRESSURE TRANSMITTERS 763, 764 This file, which was in the process of ccmpletion during previous audits, is based on Barton Test Report R3-763-6, September, 1982. A review of this file revealed that postulated plant environmental conditions have been enveloped, with adequate explanation of test anomilies. This file shows that qualification work on this item has been an ongoing process, with calculations and test reports added to the file from 1981 to 1983.
It is concluded that this file is technically adequate, and shows proper file maintenance.
3.0 CONCLUSION
This audit reviewed four files out of fifteen files affected by the HELS reanalysis. One file was reviewed to check comoletion of orevious commitments. It is concluded that the audited files showed adequate technical quality, evidence of proper file maintenance, and that prior commitments were properly resolved.
It is further concluced'that'the'

Review Continuing During the week of January 16 thru January 20, 1984, the NRC staff and its consultants from EG&G Idaho audited the equipment qualification files of Perry Nuclear Power Plant (PNPP) Units 1 and 2.
The staff and its consultants audited a total of 10 files that included 8 different speci-fication numbers.
All files were for equipment located in a harsh environ-ment.
As a result of the audit, the following are observations and comments made by the staff and its consultants at an exit interview held at the Perry site on January 19, 1984 (List of attendees attached).

The System Component Evaluation Worksheets (SCEW) do not always agree with test report data.
Consequently, the applicant must review all SCEW sheets in the equipment qualification (EQ) program to insure that all information is accurately extracted from test reports.
2.
The applicant's EQ program should include SCEW's for both quali-fied equipment and equipment to be qualified. The SCEW's for equip-ment to be qualified should list the current status and the quali-fication requirements.
3.
For equipment items where thermal equivalence analyses were used, but not documented, the applicant must document those analyses and include that documentation in the applicable files.

The applicant must review the PNPP EQ program to identify all files that include only test report summaries.
For the identified files, PNPP should obtain the applicable test report (s).
If test reports cannot be obtained, a test report summary may be acceptable, provided that the test report is reviewed and summarized by the applicant to assure proper qualification for use at PNPP, and that the necessary arrangements are made to maintain the test reports in accordance with the requirements of 10 CFR 50.49 paragraph (j).

The specific location of each piece of equipment in the PNPP EQ program must be identified.
6.
The applicant must review the PNPP EQ program to identify all equipment that has an accuracy requirement.
Both the required accuracy and the accuracy demonstrated during qualification must be documented in the EQ files.
The following comments are specific to individual specification numbers (Spec. No.) as indicated.
However, the applicant must update all files to incorporate these comments where applicable.

(Spec. No. 506-00-01) Siemens Allis Motor Summary test report is insufficient to demonstrate qualification.
This is, and will remain an open item until qualification can be demonstrated and documentation maintained in accordance with the requirements of 10 CFR 50.49.
The applicant committed to comply with the requirements of 10 CFR 50.49.
(Spec. No. 594-13) Agastat 7012 Relays Applicant committed to conduct a thermal equivalence analysis for this equipment and to include, in this file, that analysis and other docu-mentation necessary to demonstrate environmental qualification.
(Spec. No. 594-00-12) Gould J10 Relays This file did not include a test report, but it did include a test report summary.
Consequently, the applicant committed to comply with comment number 4 above and to conduct the following:

Pressure, temperature and humidity analysis of the NEMA 12 enclosure to demonstrate qualification.

SCEW revision to correct radiation and pressure data.
An analysis to show that operability above 50 C will not be necessary.

Similarity analysis to show similarity between the tested and installed models.
(Spec. No. 596-00-01) Weed Model 611 RTO SCEW must be revised to reflect data in test report.
This file should also be updated to include calculations for qualified li fe.
(Spec. No. 604-00-03) Rosemount Pressure Transmitter SCEW must be revised to reflect the radiation levels shown in test report.
(Spec. No. 563-00-02) Westinghouse Electrical Penetration The Applicant should confirm that all instrumentation circuits inside the drywell are terminated by the use of splices rather than terminal blocks.
This is applicable to all penetration assemblies use for instrumentation inside the drywell.
In addition to the seven files noted above, the following were also audited.
