INTRODUCTION In accordance with Limerick Generating Station Technical Specification 6.9.1.8, this report summarizes the Effluent Release Data and Radiation Doses to Members of the Public from Limerick Generating Station for the period December 22, 1984 I

through December 31, 1984.

m There were no measurable releases of radioactive gaseous g

effluents during the report period and therefore no resulting offsite doses to members of the public from this pathway.

There were no solid waste shipments during the report period.

EXPLANATION OF LIQUID EFFLUENT TECHNICAL SPECIFICATION 3.3.7.11 NON-CONFORMANCE (L.E.R. NO.84-018)

I In accordance with Action b of LGS Technical Specification 3.3.7.11 the following explanation of why the radioactive liquid I

radwaste effluent monitoring instrumentation channel inoperability was not corrected within the time specified in ACTION 100 of Technical Specification 3.3.7.11.

The following explanation of the cause of the event and corrective actions have been submitted with L.E.R. No. 84-018:

Cause of the Event:

The cause of the event was due to misinterpretation of the I

radwaste control room annuniciators for this system.

The annunciator display reads " LIQUID RADWASTE DISCHARGE, Hi-Lo Flow".

This alarm was incorrectly interpretated as monitoring I

radwaste discharge line flow rather than sample flow.

Operator investigation in response to the alarm detertined that it was functioning properly, incorrectly postulated an alarm failure relative to radwaste discharge flow.

The violation of the I

technical specification was the result of operations personnel not recognizing that radiation monitor SE-63-009 was out of service.

Corrective Actions:

I As a corrective action, procedural changes have been taken to clarify the alarm statement and response to the alarm:

(1) The annunciator window in the radwaste control room was revised to indicate sample pump HI/LO flow.

(2) Procedure CH-1017, " Procedure for Preparation and Control of Liquid Radwaste Discharge Permits," was revised to require a signoff by the Radwaste Operator to ensure that the radiation sample rack (including the sample pump and the radiation I

monitor) is in service per S26.1.L, " Placing the Radwaste Effluent Radiation Monitor in Service."

These corrective actions are expected to prevent similar occurrence.
