Dear Mr. Denise:
This letter provides the Kansas Gas and Electric Company (KG&E) response to your letter of December 12, 1984, which transmitted Inspection Report 50-482/84-27. As requested, the violation identified in the Inspection Report is being addressed in three parts:
a) Corrective steps which have been taken and the results achieved; b) Corrective steps which will be taken to avoid further violations; and c) The date when full compliance will be achieved.
VIOLATION 482/8427-02: FAILURE TO OPERATE PLANT BY PROCEDURE Finding 10 CFR Part 50, Appendix B, criterion XI states, in part,
. that all testing required to demonstrate that structures, systems and components will perform satisfactorily in service as identified and performed in accordance with written test procedures which incorporate the requirements and acceptable limits contained in applicable design documents." Startup Administrative Procedure ADM 14-200, Rev. 7, paragraph 4.3.2.2, "Preoperational Test Program Impicmentation", states in part, " Individual procedure steps within these subsections will be performed in the order in which they appear."

Denise KMLNRC 85-015 January 11, 1985 Contrary to the above, during performance of Prooperational Test Procedure, SU3-ALO2, Revision 0, dated April 4,1984, titled Auxiliary Feedwater Turbine Driven Pump and Valve Preoperational Test, the system startup engineer directed the operator to close the turbine steam supply valve instead of closing the pump dis-charge valves to the steam generators as required by the test procedure in Step 7.4.19.
This action constitutes a failure to perform testing in accordance with written test procedures.
Responses a) Corrective steps which have been taken and the results achieved:
The system startup engineer has indicated that he believed he was approaching an unsafe condition during this test.
Consequently, he took actions outside of the test procedure to immediately terminate the test and protect plant equipment.
The system startup engineer identified this occurrence as a Test Discrepancy and made appropriate chronological log entries. Subsequently, the appropriate test conditions were re-established and test procedure Step 7.4.19, and subsequent steps, were successfully completed.
b) Corrective steps which will be taken to avoid further violations:
KG&E Management has met with each system startup engineer and emphasized the requirements for procedural compliance.
Each system startup engineer has been required to pass a written examination emphasizing the administrative controls portaining to testing. Additionally, direct involvement in testing activities by Startup supervision and the plant operating staff has been significantly increased, c) The date when full compliance will be achieved:
Full compliance has been achieved.
