Potential Deficiency 55-83-08 10CFR50.55(e)

Damage to Guard Pipe Bellows Assemblies

Dear Mr. Keppler:

On June 27, 1983, Illinois Power notified Mr. F. Jablonski, NRC Region III (Ref:

IP Memorandum Y-17131, 1605-L, dated June 29, 1983) of a potentially reportable deficiency per 10CFR50.55(e) concerning construction damage to guard pipe bellows assemblies.

This initial notification was followed by four (4) interim reports and one (1) final report (Ref:

IP letter U-10077, D. P. Hall to J. G. Keppler dated August 16, 1983; IP letter U-10113, D. P. Hall to J. G. Keppler dated December 15, 1983; IP letter U-10141, D. P. Hall to J. G. Keppler dated April 12, 1984; IP letter U-10199, D. P. Hall to J. G.

Keppler, dated August 22, 1984 and IP letter U-10216, D. P. Hall to J. G. Keppler dated October 31, 1984).

This letter is submitted as an amendment to the final report regarding this potentially reportable deficiency.

The purpose of this amendment is to clarify the corrective action being taken to rework / repair the ten (10) damaged bellows associated with this issue.

Paragraph six (6), on page two (2) of Attachment A states the following:

"Two (2) of the ten (10) bellows will require rework.

Assembly IFWO4MB will have the small hole patched / welded (NCR 5631).

Assembly IFWO4MA has an arc strike that will be blended / buffed out."

The above stated corrective action was based on preliminary information and was incomplete in that it did not address the corrective action associated with the eight remaining damaged bellows.

The referenced paragraph should have read:

"Two (2) of the ten (10) bellows will require repair.

Assembly IFWO4MB will have the small hole patched / welded (NCR 5631).

Assembly 1FWO4MA has an arc strike that will be blended / buffed out.

Mr. Jcmec G. Keppler assemblies will be inspected and repaired as required, by removing all raised metal and sharp edges by rubbing lightl with.400 grit emery cloth as recommended by Pathway Bellow'ys Report No. 6049-001."

The. discrepancy in=our final' report resulted when the report was finalized based on oral information obtained prior to receiving and reviewing the Pathway Bellows test report and prior

.to final approval of the NCRs involved.

Final approval of the NCRs involved was withheld by Baldwin Associates' Quality review group, pending review of the official test report.

The discrepancy between the NCR dispositions and the recommendations of Pathway Bellows was discovered during the review of the official test report.

The NCR dispositions were revised to agree with the recommendations of Pathway Bellows.

To preclude recurrence of this type problem, reports concerning 10CFR50.55(e) issues-will not be finalized until associated documents have been approved and verified for agreement with the required corrective action.

It is anticipated that the repair to the bellows will be completed by December 31, 1984.

Our safety significance. evaluation, as stated in our final report, is not affected by this amendment and therefore remains unchanged.

We trust that this amendment to the subject final report provides sufficient information to clarify the overall corrective action taken to resolve this potentially reportable deficiency.
