Dear Dr. Murley:

This is to confirm my telephone call reporting suspected manufacturing and/or design defects on generators.

1.

Problem discovered at:

Long Island Lighting Company Shoreham Nuclear Plant Specification:

11907577 Colt Purchase Order Number 443570CO#19 Louis Allis Serial Numbers 700513-R1, -R2, and -R3 2.

Defect:

a.

Cracked veld in coil guards - conical baffle. Welds are intermittent velds, one inch in length, 4 inches vide - an estimated thirty to forty veld segments per coil guard.

b.

Location of cracks:

R1 - Two broken velds on drive end. One at the split between the upper half and lower quadrant sections. One at the split between the two lower quadrant sections.

R2 - Four broken welds on pedestal end. First four successive ucido immediately adjacent to the split between upper half and lover quadrant sections.

R3 - One broken veld on drive end in upper half at approximately one o' clock.

All velds were made to weld procedures and by welders both of whom were qualified to ASME, Code IX.

The velds were all visually inspected by Louis Allis.

The velds on R1 were inspected by Colt Industries. There is no record of defects during fabrication for this characteristic. All units were final inspected at Louis Allis Beloit Pouer Systems Division before shipment, including veldments. Again, no record of weld cracks exists.

Possible Cause: Unknown at this time, kbOO 2

Possible problem with other generators of the same generic design:

Sixteen units of similar design have been manufactured by Louis.

Allis Beloit Power Systems Division over a period of five years.

This is the first reported incident involving a cracked weld. However, as a precaution, Louis Allis is recommending that four generators be field inspected visually to see if there is any need for further action and concern.

Action:

a.

Repair the cracked welds discovered at LILCO. LILCO has a copy of the weld procedure WPS-402-700510, Revision 0 and vill arrange to perform the repair, b.

Arrange to have the same welds inspected on the four generators on order 700512, shipped just previous to the failed units. The four units on order 700512 are located at Public Service of Indiana, Marble Hill project.

Any further action would be contingent upon the results of step c.

b. above.
