DOCKET NUMBER 50-483 CALLAWAY PIANT FAILURE OF WESTINGHOUSE STEAM GENERATOR TUBE MECHANICAL PLUGS

- 2) ULNRC-2447, dated July 24, 1991 NRC Bulletin No. 89-01, Supplement 2 required licensees to repair or remove generator mechanical tube plugs fabricated from various heats of Inconel 600 material.
Our response to the bulletin indicated we would remove 37 of the subject plugs in the just completed Refuel 5.
Several attempts were made during the outage to remove the plugs, unfortunately none were successful.
As a result another remedial measure had to be used.
This process involved
" spot-facing" the tube /tubesheet and installing a welded plug made of Inconel 69C material below the mechanical plug.
The spot-facing was performed ur.ng a cutter which bore out the tube / plug for approximately 3/8" and simultaneously removed.010.020" of cladding from the tubesheet to prepare it for welding.
The welded plug was then installed (See Attachinent).
B&W Nuclear Services had previously qualified the process and the work was performed to their approved procedures.
We believe this is an appropriate remedial action as required by NRC Bulletin 89-01, Supplement 2, in lieu of replacement of the plugs.
