NATURE OF THE DEFECT OR FAILURE TO-COMPLY. AND THE SAFETY HAZAED URICH

}

IS CREATED OR COULD BE CREATED BY SUCH DEFECT OR FAILURE TO CONFLY:'

}

Texas Utilities reported the replacement Swing Arm to be not'

[

completely interchangeable with.the criEinal part.- Factory-j' inspection of the subject part showed critical dimensions to be the same as on the original part except for the elongated j

stud hole provided for jumproved_ seating.

Dimensional evaluation i

of the valve components show no detrimental contact 6r interference cf internal parts even under the most adverse:colorance stack.up l

conditions.-_ The replacement ara is thus considered equivalent in j

fit, form and function.

t The interference. study performed for this evaluation, revealed, j

that under extreme-tolerance' stack-up, the stud / disc assembly.

may_stop-'in a partially open position against the relieved section at the bottom of the. valve throat. and not against the-i stop (see illustration). The amount of contact interference

{-

is greater in valves with arms fabricated af ter August 1981.:-

i Should this extreme situation occur, there is sufficient flow i

area in the body to allow full flow through'the-valve. The

- more likely situation-is. that the disc / stud assembly will rotate in the bushing under impingement flow forces.. This l

rotation' removes any interference that may occur between the

[

atud and body and allows the valve to fully open.

l i

THE DATE ON WHICH THE INFORMATION OF SUCH DEFECT OR FAIu!RE TO COMPLY l

WAS ORT 1INED:

The incident was initially reported to 4W/IP International' on 16 January 1992 Additional,information has' subsequently F

been supplied by Texas Utilities,

i l

SUPPLIED FOR, OR BEING SUPPLIED FOR'ONE OR.MORE-FACILITIES OR ACTIVITIES SUBJECT TO THE REGUIATIONS IN THIS PART:

- Investigation of the subject components has' indicated no defects:

or icilure to comply.-nor has BW/IP received'any notification of-fag ur s in like components in cperating plants with many years of service (Sab h hibit A).
