This is the final written report concerning Potent i al Significant Deficiency 84-11, "QC Acceptance of Undersized / Incomplete Welds on HVAC/ Pipe Supports." DLC has determined that the deficiencies would not have adversely af fected the safety of operations of BVPS-2 and that this issue is not reportable with respect to 10CFR50.55(e).

SUMMARY

Welding audits at BVPS-2 ide nt if i'.d inspection errors with reference to welding on HVAC/ Pipe suppo rt s.

Subsequent investigation indicated that the problems involved three inspectors.

One inspector had been assigned to the inspection of pipe supports.

The other inspectors performed inspections of HVAC supports.

One ins pector had resigned prior to discovery of these de ficienc ies.

Another inspector has been terminated from the proj ect.

The certifica-tion on the third inspector was suspended on October 5,1984.

Evaluation of this inspector has been completed, and his certification was rein-stated on November 15, 1984, following retraining and interviews with SQC management.

IMMEDIATE ACTION TAKEN A program was initiated to have all HVAC and pipe supports accepted by these three inspectors subjected to 100% reinspection.

This activity is completed.

Mr. S. D. Hall, Acting Manager of Duquesne Light Compa'ny's (DLC) Regula-tory Affairs Department, notified Mr. Lowell Tripp, of the NRC's Region I Of fice by telephone on October 5, 1984, that DLC considered this matter to be potentially reportable under 10CFR50.55(e).

Further information was transmitted to Region 1 in DLC's letter (2NRC-4-182) dated November 5, 1984.

DESCRIPTION OF THE DEFICIENCY During DLC and SWEC audits, it was noted that three SQC inspectors had accepted welds on HVAC supports and pipe supports that did not neet weld requirements specified by engineerig and ins pect ion documents.

The deficient welds that were accepted did not conform to specified weld size, length, orientation, and surf ace condition requirements.

ANALYSIS OF SAFETY IMPLICATION The HVAC supports or pipe supports originally inspected and accepted by q

each of the three subject SQC inspectors have been re ins pect ed.

The supports that were ident if ied as containing. welds that were deficient have also been evaluated to determine the safety implications of the noted deficiencies.

This evaluation. indicates that all of the deficient supports would have maintained their structural integrity and performed their intended safety functios under all applicable loading combinations.

CORRECTIVE ACTION TO REMEDY DEFICIENCY i

As described previously in this response, the supports accepted by the three Inspectors involved have been reinspected.

In addition, DLC-SQC has assigned four QC Inspectors to verify the condi-tion of accepted pipe and HVAC supports, three being assigned on October 10, 1984 and one on November 26, 1984.

This reinspection was applied to 195 supports as of November 29, 1984, and nine defects have been revealed by this reinspection.

However, it is emphasized that eight of those defects occurred at the initiation of the program in October.

The November results indicate one unsatisfactory condition out of 89 supports being reinspected. The unsatisfactory condition is very minor and relates to one pore being slightly oversized.

Following this incident, all certified support QC inspection personnel have been informed of the type of deficiencies found during the reinspec-tion of the supports accepted by the three Inspectors involved, and they have been advised of the need for attention to detail.

DLC-SQC has also examined its training program related to the visual examination of welds (TCO VI B) and the specific weld inspect ion instructions related to the codes and specifications applicable to dif ferent types of supports.

DLC cons iders that SQC's training pogram was adequate, and that this has been demonstrated by the suc ces s ful results of reinspection of Ins pecto r 's work as found during the month of November 19 84.

However, DLC-SQC has initiated a revision to the training program to add more detailed instructions of how to inspect a weld.

This formal program will be available for presentation to all applicable inspectors by December 31, 1984.

With reference to the reinstatement of the certification of the Inspector referenced in Paragraph 1, the reinstatement was based on an on-the-job (0JT) witnessing of his act ivities under a special OJT training session under the direction supervision of an Assistant QC Engineer. In addition, the Inspector concerned is known to have performed many satisfactory ins pect ions in the past, in spite of this lapse that caused the errors reported. A report on the details of his OJT and interview are available at the site for review by the NRC Resident Inspectors, if required.

It should be noted that this reinstatement of certification has been made on the basis of the reinspection program now in place.

Other factors that could have contributed to Inspector error will be presented at the time of the resolution of Unresolved Item 84-14-01.
