Per the requirements of Technical Specification 3.7.12, ACTION Statement a, we hereby submit the attached Special Report concerning inoperable fire barrier penetrations, specifically a Fire Damper in the Unit 1 Switchgear Room HVAC was inoperable for greater than 7 days.

J'ENFTRATION FIRE II AltRIER SPECI Al, RF: PORT il\CKGROUND On hiay 18,1992 the fire dampers (1FD430F1, IFD524F5, IFD524F6) for the Unit 1 Switchgear Room IIVAC System were found inoperable while performing lingineering Test Pmcedure (ITFP)91-77, Fire Damper Testing. The purpose of ETP 9177 is to perform an inspection and functional test of each fire damper assembly by sisual inspection and actual closure testing. Fire  ;

dampers 1FD430F1, IFD524F5, and IFD524F6 failed to close satisfactory and were declared )

inoperable. '

As speciGed by Technical SpeciGeation 3.7.12. ACTION Statement a, a special report must be issued to the Commission pursuant to Technical Specification 6.9.2 if the fire dampers are not restored to an operable status within 7 days.

EFFECT ON UNIT OPERATION On hlay 18,1992 with Unit I defueled fire dampers IFD430F1 in the Unit 145-foot Switchgear Room exhaust, IFD524F6 in the Unit 169-foot hiain Plant Exhaust Equipment Room to Switchgear Room exhaust, and IFD524F5 in the Unit 169-foot Main Plant Exhaust Equipment Room to Switchgear Room Supply in the HVAC System for Unit 1 Switchgear Room were found disabled.

These fire dampers are addressed by Technical Specification 3.7.12. In accordance with the Technical Specification 3.7.12, ACTION Statement a, an hourly fire watch patrol was established and operability of the fin detection on one side of the affected fire barrier was verified.

In addition to the hourly fire watch patrol, the Switchgear Room is equipped with a total lboding Halon System w hich mitigates the temporary inoperability of the fire barrier.

PLANS AND SCllEDUI.ES A root cause analysis has been initiated to identify and evaluate the causes of the Gre damcar inoperability. Maintenance work is in progress to correct the problems with these fire dampers.

These dampers will remain inoperable and appropriate actions will continue in accordance with Technical Specification requirements until the fire dampers are restored to an operable status. <
