Beaver Valley Technical Specifications 4.4.5.5.a,
" Steam Generators,"
the following Special Report is submitted.
This report is required to document the results of the Inservice Inspection of the atoam generator tubes, performed during the Third Refueling Outage.
On April 6,
?.992, the inservice inspection of the tubing in steam generators 2 rcd-SG21A (S/G A),
2RCS-SG21B (S/G B), and is documentad(S/C C) wan completed.
The number of tubes inspected 2RCS-SG210 below.

of all Anservice tubes in the One hundred generators were ins)pected during the outage.
Three three steam tubes were removed from service due to tube degradation in excess of the Technica) -Specification limit of 40 pvrcent through wall.
Eight tubes were administrative 1y removed from service due t.o preservice denting.
This condition existed since fabriistion, prior to the tubes-being placed in service.
The preservice danting was located at the uppernost tube aupport plate.
Tubes administrative 1y r3 moved from service were plugged j;
with ")eak limitinga plugs on the cold leg side.
l Three Babcock Wilcox (B&W) rolled plugs on the het leg l
side. of S/G 21C were replaced with-Westinghouse Inconel 690
- plugs, completing the replacement comnitnert of all D&W plugs at Unit 2.
