Pursuant to the reporting requirements of the above reference, the subj ect code reports are submitted as enclosures to this letter for review by your office.
Enclosure 1,
" Owner's Data Repoct for Inservice Inspections" (Form NIS-1),
documents Inservice Inspection activities conducted at Cooper Nuclear Station d.tring refueling outage number fourteen (Fall 1991 Refuelint, Outage). Enclosure 2, " Owner's Report for Repairs and Replacements" (Form NIS 2), documents repair and repl, cement activities conducted during operating cycle r<.imber fourteen and refueling outage number fourteen.
This letter also serves to report an inadvertent omission on the NIS-2 report which was submitted via latter CNSS897475, f rom G. A. Trevors to U.S. NRC, dated September 13, 1989.
Code Case N-416 was invoked during replacement activities implemented via Design Change 89-023 (Maintenance Work Request 89-1688).
Reference to Code Case N-416 was inadvertently omitt ed from the NIS-2 form which documented this replacement activity (sheet 39 replacement of orifices HPCI-RO-132, 1;PCI-RO 133, and HPCI-R0134).
