REPLY TO APPENDIX A OF NRC INSPECTION REPORT 50-317/84-32: 50-318/84-32 l

We~ have reviewed the' circumstances that led to the apparent violation of Technical

. Specification 6.8 referred to by the subject inspection report. This event was caused by the failure of contractor ' personnel to fully comply with Special Work Permit (SWP) requirements.. Accordingly, the corrective actions cited below have been implemented to minimize the potential for similar violations in the future.

As acknowledged,.immediate corrective action was taken by the Supervisor Radiation Control Operations (SRCO) at the scene. The SRCO stopped all work, counselled the

contractor personnel involved and issued Radiological Control Reports to the responsible

. supervisor.. Subsequent surveys found no. personnel contamination and indicated no change in contamination levels in the area.

Improvements,in our radiation safety program have been initiated to provide better surveillance and, training of contractor personnel.

Health ' Physics -Technicians. are randomly interviewing workers in the controlled area +.o evaluate their understanding of procedures and applicable Special Work Permits.

General' Orientation Training for employees and contractor personnel, along with the annual employee requalification

. training, have been upgraded to include a realistic, graded,. training exercise utilizing a.

mock-up controlled area with postings, step-off pads, respirator, clothing receptacles,~

and Special Work Permits.

This exercise. enhances worker compliance.with instruc tions/proce dure s/direc tive s.
