SEQUOYAH NUCLEAR PLANT UNITS 1 AND 2 - NRC-0IE REGION II INSPECTION REPORT 50-327/84-21 AND 50-328/84-22 The subject OIE inspection report dated September 17, 1984 from R. C. Lewis to H. G. Parris cited TVA with two Severity Level IV Violations. Enclosed is the response to the items of violation in the subject inspection report.

ENCLOSURE RESPONSE - NRC DTSPECTION REPORT NOS.
50-327/84-21 AND 50-328/84-22 R. C. LEWIS' LETTER TO H. G. PARRIS DATED SEPTEMBER 17, 1984 Item 327, 328/84-21, 22-01 Technical Specification 6.11 states that Radiation Protection Procedures shall be written and adhered to.
RCI-3, Personnel Monitoring, states that TLDs and dosimeters shall be worn on the front of the person between the neck and waist.
Contrary to the above, the licensee did not adhere to the procedure as follows:
Although a procedure required wearing TLDs and dosimeters between the neck and waist, several individuals were observed on August 8-9, 1984, wearing TLDs and dosimeters in their trouser pockets.
This is a Severity Level IV Violation (Supplement V).

Admission Or Denial Of The Alleged Violation TVA admits the violation occurred as stated.
2.
Reasons For The Violation If Admitted Personnel failed to follow approved plant procedures.
3.
Corrective Steps Which Have Been Taken And Results Achieved A.
A memorandum from the plant manager was written and transmitted to all section supervisors on August 17, 1984 This memorandum stated the problem of incorrect wearing of personnel monitoring devices and described the correct procedure for wearing these devices.
B.
This violation was discussed at a daily staff meeting by the plant manager. All section supervisors were in attendance and the importance of wearing personnel monitoring devices correctly was stressed.
C.
Internal surveys have been performed by the plant to measure the effectiveness of the memorandum. Results of this survey indicate that performance has improved.

Corrective Steps Which Will Be Taken To Avoid Further Viointions Internal surveys will continue to be made to mnintain adherence to written procedures.

Item 327, 328/84-21. 2?-02 Technical Specification 6.11 states that Radiation Protection Procedures shall be written and adhered to.
SQA-133, Radioactive Waste Management, states that green containers shall be used for nonradioactive material and yellow containers for radioactive material.
Contrary to the above, the licensee did not adhere to the procedures as follows:
Although a procedure required that yellow containers be used for radioactive materials, yellow containers were observed on August 8-9, 1984, being used for nonradioactive material.
This is a Severity 1.evel IV Violation (Supplement V).
