The. prerequisites stated in your letter, Log:
NOV-00424 of March 15, _1990, for the operation of the FAVA Microfiltration System have i
.been verified.
The prerequisites were satisfied as follows:
1.
FAVA system operating procedure T-0PER-90-1, Revision 2,
Step 2.1.5 requires "An operator should be in attendance at the system controls for the entire duration when the sytem is operating."
2.
All hoses going to and from the system were checked as properly installed by Temporary Modification #A90-001 and hydrostatically tested to the requirements of Regulatory Guide 1.143 by MWO
- A9000081.
Both the installation checks and the hydrostatic test were satisfactory.
3.
FAVA system operating procedure T-OPER-90-1, Revision 2,
Step 3.5 requires, as a prerequisite, "The top cover is securely fastened to the FAVA Filter Skid."
In addition to the above requirements, the demineralizer operating procedure (13290-c) which is used in conjunction with the FAVA Oystem operating procedure requires "The Alternate Radwaste Building (ARB) roll up door must remain closed while process equipment is operating unless the ARB is continuously manned and a Continuous Air Monitor is in operation."
