Reference 1 requested that the requirement to submit ALAB reports be deferred until such time as the project is restarted. We acknowledge that Reference 2 advises us that relief from this reporting requirement should be pursuant to a 10 CFR 50.90 filing. 13 NONCONFORMANCE Contrary to the above, the subject procedures were not approved as of Jcnuary 11, 1985.
These procedures address safety related activities that are ongoing 4
during the Shutdown Phase of the Midland Energy Center.
It is recognized that there have been extenuating circumstances such as:
limited resources, slow response time during comment cycle, and higher priority for Shutdown activities that affect the Company's financial condition.
It is also recognized that these procedures will document, not change, the methods by which these activities are parformed.

It was determined that soil boring logs B1, B2, B3 and B4 may be invalid because of an j
error that occurred during the submittal of boring data obtained by the subcontractor-Raymond Company. This data was used in the evaluation that determined the potential for soil liquefaction of the sand layer at elevations 600.5' and 603', which is now determined to be inconclusive.

The subject NCR was generated during Receipt Inspection of a shipment of a grease product manufactured by Mobil Oil Corporation. The specification required grease designated as Mobilux 2 to be shipped while the actual shipment contained Mobilux EP-2 grease.
Mobilux 2 and Mobilux EP-2 are lithium soap based gresses of the same grade.
EP-2 also contains an additive that makes it suitable for use in " Extreme Pressure" applications.
Based on the industrial products brochure describing the Mobilux 2 and the Mobilux EP-2 lubricants and discussions with Mobil Oil personnel, we have concluded that in all cases where Mobilux 2 grease is i
specified, Mobilux EP-2 may be substituted.

In accordance with the requirements of 10 CFR part 21. Ruskin Manufacturing l
l Company, located at 3900 Dr. Greaves Road, Grandview, Missouri 64030, is herewith reporting that we have discovered a deficiency with equipment furnished by Ruskin Manufacturing Company for Nuclear Power Plant application.
On August 7,1984 Ruskin received a letter from The Waldinger Corporation addressing seven (7) fire dampers failure to close under normal duct pressure at Palo Verde Nuclear Generating Station.
On August 13, 1984 Ruskin visited the jobsite to inspect dampers. The results of the insoection revealed damper condition to exhibit bent blades, bent blade tracks, rust and zine rich paint on springs.
On August 20, 1984 after Engineering review of inspection results, Ruskin again visited the jobsite for Engineering review of above referenced damper conditions.
As a result of this review Ruskin recommended that the blade packages be replaced i
iti the effected fire dampers.
On October 30, 1984, Ruskin was notified, by The Waldinger Corporation, that the new blade packages had been installed and tested. The fire dampers with the newly installed blade packages would still not cycle under normal duct pressure.
Ruskin Engineering immediately began lo evaluate the problem to determine the cause of the daipers failure to close under air flow.
Results of this evaluation.
indicated the test methods originally utilized to test dampers under flow may not accurately depict actual installed conditions.
Ruskin fire dampers were tested in accordance with AMCA 500, test figure 5.5, counted on the face of the test tunnel.
Actual installed condition, in ductwork, may not result in proper performance as tested to AMCA.
Ruskin, on November 2, I
and 51984, performed air flow tests in accordance with AMCA 500 with damper mounted on face of tunnel as well as with damper mounted in ductwork.
The results of this test indicated that face mounted damper would close at much higher static pressure than the damper which was installed in'ductwork.

I since no facility exists to properly test dampers under flow that would simulate all field installed conditions, and relationship between face mounted damper and damper installed in ductwork cannot be accurately calculated, Ruskin recommends

that fire dampers supplied with closure springs, which require closure 'under air flow conditions, be tested to verify proper operation. This condition affects only interlocking blade fire dampers Model IBD-21,1B0-23 and NIBD-23 supplied with closure springs by Ruskin.
l Ruskin is currently researching methods to modify fire dampers to ensure closure i
under air flow.

In accordance with the requirements of Title 10,' Chapter 10, Code of Federal Regulations, Part 21, Transmarica Delaval, Inc., here-by notifies the canniasion of a potential defect in a companant of a DSR or DSRV Standby Diesel Generator. 1here could exist a tial problan with the Air Filter in the hine Ccritrol Panel.poten-At Cleveland Electric Perry Nuclear Station, it was recently dis-covered that the Air Filter for the Engine Control Panel had a polycarbonate transparent bowl. On November 5,1978 the Filter purchase specification taas changed from the polycarbonate trans-parent towl to a metal bowl. The polycarbonate bowl is ficiFrated 150 psig at 125'F and the metal bowl is rated 250 ps at! 175'F.
Een the specification was originally issued, the V rated the posycarbonate bowl at 250 psi. The air pressure idtich the Filter will see in operation cycles between 200 and 250 at room tenparature.
New metal bowl Filters have been sent to Cleveland Electric to re-place the polycarbonate bowls.
We will send copies of this letter to all sites tdiere we furn4=h=A the Engine Control Panel with a s4=414=* Filter so that the sites can verify that the Filter has a metal bowl.

References 1 and 2 provided additional information on this subject.
Pending resolution of our request to waive the requirement to submit a written 30-day 50.55(e) report during the project shutdown, we have prepared this interim report on the above subject.
I Description of Concern It has been determined that the data for Borings B-1, B-2, B-3 and B-4 submitted to the NRC by references 3 and 4, presented in the Midland FSAR and in the response to 50.54(f) questions, do not represent the actual borings l
which were taken at the diesel generator fuel oil tank location.
Reference 2 is attached to provide a description of the situation and the concern raised as to the potential effect on analysis of liquefaction potential of the soils beneath the diesel fuel oil tanks.

It is our conclusion based on a conservative assessment of available information that the analysis of the liquefaction potential of the soils beneath the diesel fuel oil tanks is inconclusive.
Root Cause It appears that a combination of events occurred which allowed the use.of incorrect boring logs.
1.
In June and July of 1977 respectively, the same company (Raymond) made the actual diesel fuel oil tank borings and the borings which were erroneously submitted. The borings which were mistakenly submitted related to an AT&T Project in Midland County that had no relationship to Bechtel or Consumers Power Company. The actual B-1 through B-4 borings at the Midland jobsite were taken to obtain soil samples at the proposed location of the diesel fuel oil tanks.

Both boring series were labeled B-1 through B-4 and the soil types, boring depths and taking of Shelby tube samples for the AT&T borings were, in general, similar to other borings taken at the Midland site. The AT&T i
borings listed the job address as Midland, Michigan.

During the Site-wide soils investigation in the spring of 1979, it was determined that boring data was required in the area of the diesel generator fuel oil tanks. Bechtel personnel then recalled that borings i
had been made there in the summer of 1977. The Bechtel Geotechnical l
Engineering Group (Geotech), which was not involved in the 1977 borings, asked Project Engineering to provide the boring logs. Project Engineering believed that no drilling logs had been made for the B-1 through B-4 l
borings at the Midland Site. However, their response to Geotech apparently l
did not clearly convey this but rather it was understood by Geotech that l
Project Engineering didn't have any logs and not that they didn't exist.
l Bechtel speculates that the handwritten drilling logs taken for the AT&T project may have been supplied by the drilling contractor (Raymond) in response to a request by Geotech for logs of the B-1 through B-4 borings taken at the Midland jobsite.

In March of 1982 the handwritten B-1 through B-4 drilling logs again surfaced in conjunction with the design of the site dewatering system.
The recent Bechtel investigation indicates that one Bechtel employee believed that the handwritten driller's logs did not record borings taken l
in the area of the diesel fuel oil storage tanks and that there may have been discussions within Bechtel's Geotech group on this matter. Apparently a decision was made based on the available data to include these driller's l
logs as input to the liquefaction analysis for the buried diesel fuel oil tanks. Subsequently, Consumers Power submitted the handwritten driller's l
logs to NRC informally and then via reference 3.

Bechtel prepared typewritten boring logs from the handwritten driller's logs for inclusion in the June 1982 FSAR submittal. This limited further j
opportunity to detect from the original driller's logs that they were incorrect l

Serial 32097 84-17 #1 as to the Midland site location since the original driller's logs were now back in the files. The typed boring log profiles include additional information which is believed to have been obtained from the test records of the borings actually taken at the Midland jobsite, such as the boring coordinates.
It is normal practice for a handwr'itten boring log to be produced at the time a boring is made. These logs are produced by the technical person overseeing the drilling operation. Information concerning boring locations is entered in the logs by such technical personnel, and thus can be directly traced to the Midland jobsite.
In the case of Borings B-1 through B-4, the erroneous logs (ie, the logs of the AT&T borings) contained no location coordinates or other information directly indicating the location of the borings. Instead, the location information obtained from testing data concerning the onsite B-1 through B-4 borings was assumed to be applicable to the logs of the offsite borings. As previously stated, this confusion resulted in logs of the incorrect borings being submitted to the NRC.
The absence of location information on the drilling logs traceable to the Midland site is a circunstance unique to Borings B-1 through B-4.
A review of all Bechtel borings incuded in Section D.1.4 of the Soils 50.54(f) response indicates that borings B-1 through B-4 are the only borings cade from a driller's log. All the other borings were produced from handwritten boring logs by a Bechtel Geotechnical Engineer assigned to the t
drilling. The nomal practice of involvement by Geotech personnel would l
preclude any generic repetition of the unique circumstances of this l
particular situation.

1 The controls in place at the time of the Project shutdown were adequate to prevent a recurrence of this problem, thus no process corrective action is necessary.
Part corrective action required includes correcting the affected documentation l
and determining if additional analysis or additional field information is l
necessary to address the issue of potential liquefaction of soils below the diesel fuel oil tanks.
Because of the Project shutdown, these corrective actions will not be pursued and no further reports on this subject will be made unless the Midland Energy Center is reactivated. This item will be entered in our Correspondence and Commitment Management System for long-term tracking.
