l IIACKGROUND On July 20, 1992, fire damper [IFD524F1, IFD524F2, 2FD526F2, and 2FD526F3 were found INOPERABLE while performi hg testing in accordance with criteria set forth in CCI-l17 Temporary ModiGeation Control Attachrnent TS-1, Troubleshooting Control Form. Fire dampers 0FD512F1 and 0FD512F2 were subseq~ ently declared INOPERABLE as they were installed in the same manner as the ebove refere iced dampers but were inaccessible for testing purposes. The above referenced dampers were a concern because similarly installed dampers, tested as pait of Engineering Test Procedule ETP) 91-77, failed to close completely due to incorrect installation.

The purpose of ETP 91.-h w(as to perform an inspection and functional test or each assembly by visual insp.,ection and actual closure testing. Fire dampers IFD524F1, IFD524F2, 2FD526F2, and 2FD526F3 failed to close completely. As a result, all were declared inoperable.

As required by Tech nical Specification 3.7.12, ACTION Statement a, a Special Report must be issued to the Comr nission pursuant to Technical SpeciGcation 6.9.2 if the fire dampers are not restored to an opevable status within seven days.

I EFFECT ON UNIT OPERATION l On July 20, l'392 with Unit I at Mode 5 and the Unit 2 at Mode 1. lire dampers IFD524F1 (Unit 1 Auxiliary Byilding Exhaust), IFD524F2 (Unit 1 Containment Purge Exhaust),2FD526F2 (Unit 2 Auxiliary IVuilding Exhaust) and 2FD526F3 (Unit 2 Containment Purge Exhaust) were found INOPERADLE. Additionally, fire dampers OFD521F1 (Control Room Cable Spreading Room Supply) an!! 0FD512F2 (Contul Room Cable Spreading Room Exhaust) were also declared INOPERAItLE 3 as their OPERABILITY could not be veiified due to inaccessibility. These fire dampers arp addressed by Technical SpeciGeation 3.7.12. In accordance with Technical Specificatiort 3.7.12, ACTION Statement a, an hourly fire watch patrol was established and OPERABIL,lITY of fire detection instruments on one side of the affected barrier was veriGed.

A root cause analysis was conducted and has identified the causes of fire damper INOPERABILITY.

The root cause and corrective actions for this event were discussed in LER 92-004. Engineering and Maintenance work is in progress to correct the problems with these fire dampers. These dampers will remain INOPER ABLE and approntiate actions will continue in accordance with Technical SpeciGcation requiremcms until the fire dampers are restored to an OPERABLE status.
