A2. I am employed by Brown & Root at Comanche Peak Steam Electric Station in Glen Rose, Texas.

Q3. What is your title?

A3. Quality Control Engineer in the N-5 Code Data Group.

Q4. I show you a two-page document marked as Matheny Exhibit 1 and ask you to identify it.

A4. This is a current copy of my resume.

05. Have you ever supervised Jack Stanford?

A5. Yes, I supervised Mr, Stanford for about a year, up until around last March.

06. Are you familiar with a weld data card on which Mr. Stanford corrected the dates or his inspection in January, 1984, which has been the subject of testimony in this proceeding?

A6. Yes, I am.

Q7. Did you work on January 14, 19847 A7. Yes, I did.

08. What day of the week was that?

A8. A Saturday.

09. Do you know whether Mr. Stanford worked on Saturday, January 14? ,

A9. Yes, he did.

Q10. What was Mr. Stanford's work assignment that day?

A10. He was providing QC coverage for a welding crew or crews supervised by Al Justice.

Oll. Did you have any conversations with Mr. Stanford on that day?

All. Not that I recall.

Q12. Did you have any conversations regarding welds on System AF-1-SB-007 on that Saturday?

A12. Yes, I did. On Saturday morning, I had a discussion with Mr.

Justice, who is a General Fore. nan in the Pipe Department, and Mr. Pitts, who is in Mechanical Engineering.

Ol3. What did you discuss with Mr. Pitts and Mr. Justice?

A13. Resolving a problem with Field Weld 40C. Mr. Justice wanted to weld on FW-40C but there was an open non-conformance report against it.

Q14. Please describe the NCR.

A14. On January 3, 1984, Laurel Yates wrote the NCR against FW 40-C for a minimum wall violation.

015. Mr. Matheny, I show you this seven-page package of documents marked for identification as "Matheny Exhibit 2" and ask if you can iden.ify page 4 of 7.

A15. Yes. This is the NCR written by Laurel Yates that I discussed with Mr. Justice and Mr. Pitts on Saturday, January 14.

016. Please refer to page 5 of 7. Can you identify that document?

A16. This is the Weld Data Card for FW 40C showing Mr. Yates' "UNSAT" of the cleanliness hold point.

Q17. Does the Weld Data Card indicate that Ms. Yates wrote an NCR in connection with the UNSAT hold point?

A17. Yes. There is an asterisk next to the "UNSAT" hold point, and down below, a reference to NCR No. 12382.

018. Was the problem that Mr. Justice and Mr. Pitts discussed with you resolved?

A18. Yes, it was. Rev. O of the NCR had been dispositioned to weld repair the minimum wall violation. Mr. Pitts said that he had written a DCA on Friday, January 13, to accept the minimum wall violation based on stress analysis, but he could not obtain the final signature on the DCA until Monday. We decided to revise the NCR with a partial disposition. The disposition was that the DCA would be issued to accept the I

minimum wall violation and that final NDE would would not be performed until closure of the NCR. This would allow Mr.

Justice to resume work on FW-40C. The NCR was revised on that Saturday and the WDC for FW-40C was resubmitted to ANI for review since the unsat cleanliness hold point had to be reestablished.

019. Please refer to Matheny Exhibit 2, page 2 of 7. Can you identify that document?

A19. This is Rev. 1 of NCR 12,382, showing the partial disposition of the NCR that I discussed with Mr. Justice and Mr. Pitts on January 14.

Q20. Please refer to Matheny Exhibit 2, page 6 of 7. Can you identify that document.

A20. Yes. This is DCA No. 19,600, which Mechanical Engineering issued to accept the mimimum wall violation identified by NCR No. 12,382.

021. If Mr. Pitts wrote the DCA on Friday the 13th, why couldn't the NCR be closed on that Saturday, the 14th, instead of receiving only a partial disposition?

A21. The DCA required a final signature before it could become fully effective. Referring to the DCA, under "5.C" there is a signature line for " DESIGN REVIEW PRIOR TO ISSUE." The individual responsible for that review was not, as I recall, at the site on that Saturday.

Q22. When did this DCA become fully effective?

A22. The design reviewer approved the DCA on January 16, and it appears that the Document Control Center received the DCA on that day. When copies of the DCA are available from DCC, then the DCA is in effect.

023. Was NCR No. 12,382 finally closed?

A23. Yes. It was closed on January 17, 1984.

Q24. Please refer to Matheny Exhibit 2, page 1 of 7, and identify that document if you can.

A24. This is Rev. 2 of NCR 12,382, showing closure of the NCR.

025. Where does Rev. 2 reflect closure?

A25. The last signature line in the lower left hand corner of the NCR is for closure. It was signed by the QA NCR Coordinator on January 17, 1984.

Q26. When did you first become aware that Mr. Stanford had corrected the dates on the WDC for FW-40C?

A26. About a week or so after January 17, when Sue Ann Neumeyer brought me the Weld Data Card.

Q27. What was the substance of your conversation with Ms.

Neumeyer?

A27. Ms. Neumeyer thought there was a problem with Mr. Stanford lining through the 1-14-84 date and entering 1-17-84. I told her that I would send Mr. Stanford to talk to her when he came in from the field.

Q28. Had you discussed this matter with Mr. Stanford prior to that time?

A28. No, I hadn't.

029. What did you do as a result of your conversation with Ms.

Neumeyer?

A29. Nothing. I thought Mr. Stanford would be able to clear up the problem.

030. Did yod hear anything further regarding this WDC?

A30. Yes, I did. Ted Blixt called me to his office and showed me an NCR that Ms. Neumeyer had written on the WDC. Mr. Blixt asked me to get Mr. Stanford for a meeting with Mr. Siever so we could get the NCR resolved.

Q31. Did such a meeting take place?

A31. Yes.

Q32. When was the meeting?

A32. As I recall, right after lunch.

Q33. Where was the meeting?

A33. In Mr. Siever's office.

Q34. Who attended the meeting?

A34. I attended several meetings on this NCR over the course of one day, or perhaps two days. I cannot recall exactly who attended each meeting, or what discussions took place at the meetings. I do recall a meeting with myself, Bob Siever, Ted Blixt, and Jack Stanford, and I remember another meeting attended by Ms. Neumeyer and her supervisor, Dwight Woodyard.

I cannot, however, remember the sequence of these meetings.

035. Do you recall whether Mr. Stanford explained the corrected dates on the Weld Data Card?

035. At one meeting, Mr. Stanford was asked why he lined out the date of the 14th and entered the 17th on the WDC hold points for final V.T. & P.T. He said he couldn't remember why. Mr.

Siever asked Stanford if he had performed a P.T. on FW-40C on the 14th. Mr. Stanford said he wasn't sure. Mr. Siever then asked me to find a P.T. report for FW-40C for January 14th.

I went through the files but could not locate a report for the 14th. But I did find a P.T. report signed by Stanford dated the 17th for FW-40C. Later in the day, Mr. Siever and Greg Bennetzen and myself were in Mr. Stever's office when Mr. Stanford came in and told us that he remembered the events of FW-40C.

Q36. What was Mr. Stanford's explanation?

A36. He said that the weld was not completed when he left work Saturday, January 14. He said that it wasn't completed until 1 or 2 a.m. on Sunday morning, the 15th. Stanford said he had no further involvement with FW-40C until the 17th, when he was asked to do a final V.T. and P.T. According to Stanford, Robbie Duncan was with him when he did the final NDE on the 17th. Stanford explained that as he was signing the hold points he signed the 14th in error and lined through it and er.tered the 17th. He said Mr. Duncan could verify his description.

Q37. Did you speak with Mr. Duncan?

A37. Yes. I left the meeting and found Mr. Duncan, and asked him to come with me to Mr. Siever's office. When we got there, Mr. Duncan confirmed that he and Stanford had performed the NDE on FW-40C on January 17. He also described his knowledge of Mr. Stanford's making errors in dating the V.T. and P.T.

hold points on the WDC.

Q38. Did you take any further action regarding this matter?

A38. When Mr. Duncan left the office, Mr. Siever instructed me to prepare a brief statement summarizing the problem with the WDC's changes in dates, and Mr, Stanford's explanations, for Stanford and Duncan to sign. Because it was then the end of the work day, I didn't write the statement until the next day.

039. I show you a document marked Matheny Exhibit 3 and ask if you can identify it.

A39. Yes. This is the statement that I prepared for Stanford and Duncan to sign.

040. Is the text in your handwriting.

A40. Yes, it is.

041. What did you intend the statement to reflect?

A41. The first thing I did was to describe my understanding of what Ms. Neumeyer thought was the nonconforming condition. I then described my understanding of Mr. Stanford's first and later explanations of the corrected dates on the WDC.

042. When did he offer those explanations?

A42. Well, he really didn't have any explanation until he actually recalled performing the V.T. and P.T. inspections with Mr.

Duncan on the 17th and came in to tell Mr. Siever, Mr.

Bennetzen and myself about it. Before then, at the various meetings, my impression was that Mr. Stanford simply did not remember the inspection in question, or why he had corrected the dates.

043. Mr. Matheny, prior to the time that Ms. Neumever showed you the WDC for FW-40C, had you discussed the FW-40C, any inspections of the FW-40C, the WDC for FW-40C, or the corrected dates on the WDC with Mr. Stanford?

.A43. No.

044. On what day did you prepare the statement identified as Matheny Exhibit 3?

A44. On the same day that Mr. Stanford and Mr. Duncan signed it, on January 26, 1984.

045. What did you do with the statement after they signed it?

A45. I turned it over to Bob Siever.
