" Cracks in Tack Welds in Dowell Valves" On August 16, 1984, Detroit Edison's Mr. L.
P.
Bregni tele-phoned Mr. J. McCormick-Barger of the NRC Region III to report that cracks had been detected in tack welds on two valves manufactured by the Wm. Powell Company.
This deficiency is potentially reportable under 10CFR50.55(e) and is being tracked as Item 137.
Description of Deficiency A visual examination of motor operated valves V8-2159 and V8-2160 revealed cracks on tack welds holding keys in the bushing keyways.
One key is in the yoke to shaft bushing keyway and the other is the key in the shaft bushing to shaft keyway.
These deficiencies were documented on Nonconformance Report ( NCR ) 84-1282.
The longitudinal cracks extend completely across the tack weld; therefore the tack weld was still sufficient to hold the key in the keyway.
Motor operated valves V8-2159 and V8-2160 are "Y" pattern globe valves each equipped with a Limitorque motor operator, and are located in the residual heat removal system.
There are a total of six "Y"
pattern globe valves with this configuration installed at Fermi 2.
Analysis of Safety Implications Complete failure of the tack welds resulting in an unobstructed path for the key to slide out of the keyway could result in the valve being inoperable.

o All uix similar globe valves manufactured by the Wm. Powell f Company will be inspected to assure tack weld integrity.
o Repairs or modifications as required will be made to secure the keys within the keyways.
The Wm.
Powell Company was consulted and a disposition is being prepared.
Another report of this item, either interim or final, will s
be sent when further information is available.
