Dear Dr. Murley:

The attached letter has brought to our attention the problems of sane material being furnished to Limerick Generating Station with incanplete docunentation and lacking the chenical overcheck requirement of paragraph NCA3867.4 (e) (2). This deficiency has previously been reported to the USNRC by Guyon Alloys Inc. under the provisions of 10CFR21.

We consider the deficiency described in the attachment as a significant deficiency per 10CFR50.55(e) and are hereby notifying you as required.

We believe that the Guyon Alloys Inc. 10CFR21 notification letter to Bechtel Power Corporation (Agent to PECO) provides the information required for 10CFR50.55(e) reporting.

Therefore, we anticipate that this letter will be our only report to you on this subject.

We have completed our investigation into this matter and have determined that there is no objective evidence to show that any of the six (6) weldolets in question have been installed in the Limerick Generating Station.

Orders No. 8031-F-50970 8031-F-56377 Limerick Generating Station 10CFR21 Notification Pursuant to the notification requirements of the Code of Federal Regulations Title 10, Part 21 (10CFR21) per attached copy Guyon Alloys has received a notification from the Bonney Forge-Gulf & Western Manu-facturing Company advising that some material shipped from their Carlinville, Illinois plant has been furnished to Guyon Alloys with incomplete documentation and lacking the chemical overcheck require-m:nt: of Para. NCA-3867.4(a)(2).

Based on a thorough review of our records in conjunction with the msterial listing received with the Bonney Forge notification, some of tha items of material cited by Bonney Forge have been identified as hcving been in turn furnished by Guyon Alloys against the subject order (s) for the referenced p'roject.

The items so identified are listed on attached " Exhibit I.

As discussed in the attached copy of our return letter to Bonney Forge, it is suspected that the identified items have been manufactured from unqualified stock material which was incompletely or improperly qualified (upgraded) under the provisions of NCA-3867.4(e) or NA-3767.4(e) cs and if applicable.

Additional detailed information as requested by our letter will be relayed to you upon receipt from Bonney Forge.

In as much as Guyon Alloys as a material supplier does not have the ccpability of determining whether the use of the discussed material as furnished may create a substantial safety hazard, this notification and tho add 1.tional information to follow is being provided to facilitate l

For such fittings procured to an earlier Code Edition and/or Addenda, the use and acceptance of the size / quality program exclusion provisions per a later Code Edition and Addenda may be necessary.

Please advise Guyon Alloys of the results of your determination of whether the use of the fittings as furnished would create a: sub-senatial safety hazard, the status, and any proposed disposition of tho fittings involved.
