An outline of the incident: On January 4, 1996, the reactor was operated at 1MW for 8Hr. 14 Min. for a routine irradiation.

At the time for shutdown, the operator drove the four control rods in using the rod down switches. All four rods started driving in and the reactor power decreased rapidly but the operator noticed that the Shim-2 control rod stopped at 917 units, (from a last recorded operating position of 954 units on a O to 1000 unit scale.) The other rods continued to drive in until, at about 200 units, the operator scrammed the reactor.

Three rods dropped into the core. At this time the reactor was subcritical by more than $5.00, but Shim-2 rod stayed at 917 units. The operator notified the Acting Reactor Supervisor and he went out on the reactor bridge to check Shim-2 rod. The rod was at an elevated position and not in the core. After examining the rod drive he shook the barrel on Shim-2 and the rod fell into the core. The failure of the control rod to be inserted into the core upon shutdown was reported to the NRC at about 1645 Hr. on January 4, 1996.

Since the initial telephone report, the Reactor Staff has .

been in daily contact with NRC Region IV personnel. As of this l date, the Reactor remains in a shutdown condition.
