Modification No. 6 to Task Order No. 33 Entitled.
" Individual Plant Examination (IPE) Reviews - Internal Events -
Front End Only (Calvert Cliffs)" under Contract No. NRC-04-91-066 This confirms verbal authorization provided to you on November 1,1995 to commence work under Modification No. 6 to Task Order No. 33.
In accordance with Section G.5. Task Order Procedures, of the subject contract, this letter definitizes Modification No. 6 to Task Order No. 33 which: (1) correct " Modification No. 4" dated 6/26/95 to read " Modification No. 5" (2) increases the level of effort by adding 16 professional hours for continued performance of work under Task Order No. 33 due to a change in contractor personnel to complete support efforts for the NRC in the review of the Calvert Cliffs IPE submittal. and (3) increases the task order's ceiling by $2.022.00 from $27.892.00 to $29.914.00.
The obligated amount of this modification is $2.022.00.
Accordingly, the task order is hereby modified as follows:
The third and fourth paragraphs of the definitization letter of the basic task order are deleted in their entirety and the following is substituted in lieu thereof
" Task Order No. 33 shall be in effect from May 16. 1994 through i
December 31, 1995, with a total cost ceiling of $29.914.00.
The amount of $27,699.00 represents the total estimated reimbursable costs and the amount of $2.215.00 represents the fixed fee."
"The amount presently obligated by the Government with respect to this task order is $2.022.00."
