Unit 1:
There were three challenges.to the pressurizer safety and power operated relief valves.(PORV) at Unit 1
during the period of January 1, 1991, to December 31, 1991.

Date:
April 13, 1991 Plant Conditions:
Mode 4, RCS pressure was 300 psig, Overpres-sure Protection System (OPPS) was in service.
A cooldown to Mode 5 was in progress.
Summary:
Pressurizer Power Operated Relief Valve, PCV-RC-445C, opened while OPPS was in service due to RCS pressure slowly reaching 327 psig.
Pressure decreased to 303 psig and PORV reclosed.

July 4, 1991 Plant Conditions:
Mode 4, RCS pressure was 315 psig, Overpres-sure Protection System (OPPS) was in service.
A cooldown to Mode 5 was in progress.
Summary:
Pressurizer Power Operated Relief Valve, PCV-RC-445C, opened for 37 seconds then reclosed, while reactor operator was controlling RCS pressure in a narrow band to maintain RC-P-1C seal delta-pressure and seal leakoff flow.

RCS pressure was 360 psig, Overpres-sure Protection System (OPPS) was in service.
. Summary:
Pressurizer Power Operated Relief Valve, PCV-RC-455C, opened for 10 seconds then reclosed.
The cause for actuation wac unknown.
No over-pressure condition existed.
-Unit 12:
There-were no challenges to the pressurizer safety and power operated relief valves (PORV) at Unit 2 during the period of January 1,
1991, to Deceiaber 31,-1991.
