This office has received notification that your facility discharged oil or hazardous materialu in gaantitles that may be harmful, in violation of Section 311(b)(3) of the Federal Water Pollution Control Act Amendments of 1972, 33 U.S.C. Section 13 21 (b) ( 3 ) . Pursuant to Section 308(a), 33 U.S.C. Section 1318(a), you are hereby required to submit to EPA the following information. Any person who violates Section 308 is subject to a civil penalty of up to $10,000 por day of violation 33 U.S.C. $

1319(d). Further, any person who willfully or negligently violates section 308 may be punished by a fine of not less than

$2,500, nor more than $25,000 per J y of violation, or by imprisonment for not more than one year or both, 33 U.S.C.

A fuel oillug strainer drain broke open and leaked diesel __ fuel into a building sump which overflowed into the storm drain avstem

19. Describe steps taken to contain and clean up the spilled material and mitigate environmental damage.

Most of the diesel fuel was trapped in the stormwater pipe and pumped out.

The pipe was flushed and fuel / water mixture captured at the outtall OUI oll/ water separator and removed for disposal, Fuel in the canal was captured by the permanent oil booms and removed with absorbents.

20. Describe action taken or proposed to prevent a recurrence of this type of spill.

Renaired and increased preuntive maintenance of the coujynent involved.

21. List the federal and state agencien, if any, to which the owner or operator reported the discharge. Show the agency, its location, the date and time of notification and the official contacted.

This event was reported due to the large volume of fuel spilled to the v.torm ,

drain system and the potential for a large discharge, llowever, containment and recovery actions were successful and none of the fuel was discharged to the watervays. ,
