River Bend Station Unit 1 Refer to: RIV Docket No. 50-458/ Report 84-02 This letter is in response to the Notice of Violation contained in NRC I&E Inspection Report No. 50-458/84-02. The inspection was conducted at the River Bend Station construction site by Messrs. H. W. Kerch, R. H. Harris, and R. M. Campbell of the Region I office-during the period February 13 - March 9, 1984, of activities authorized by NRC Construction Permit CPPR-145 for River Bend Station Unit No,'l.

Gulf States Utilities Company's (GSU) response to the Notice of Violation 84-02-03, " Failure to Meet Radiographic Quality Requirements" is provided in the enclosed attachment. This completes GSU's response to the Notice of-Violation.

Corrective Steps-Which Have Been Taken and the Results Achieved.

General Electric issued FDDR LDIa251, Rev. 0 to Stone &

Webster for confirmatory re-radiography of the identified-welds. , Gulf-States Utilities issued-Deficiency Report DR-173

-identifying this. condition for evaluation as potentially reportable in'accordance with 10CFR50.55(e).

Thellongitudinal welds of.the 50 and 90 elbows were re-radiographed to assure. compliance with the ASME code

-requirements.- Weld ripples had been removed from the 50 elbows and the re-radiography revealed that no' indications had been' masked or confused with the imageg of the weld ripples.. The confirmatory film ~for the 90 elbows meets the density' requirements of'ASME Section V.- This radiographic tfilm was reviewed by Stone &. Webster Field Quality. Control, General' Electric, and GSU-Quality Assurance personnel.

Fullicoverage of these longitudinal welds could not be o obtained due to lugs being installed.on the welds on the g0

' elbows and sockolets installed through_the weld of the 90 elbows.

The:50 -elbows'have:two (2)' seam welds. The outside seam is
62' inches ~1ongzand the inside-seam isL41 inches long. ~There' is one ? (1) twelve' inch. lug installed over each' seam. Of the a103 totalfinches of' weld, 2411nches could not be.

re-radiographed. Tne 79 inches which was' accessible and was  ;

w re-radiographed gave coverage to 77% of the total weld.

Th'e' 90 l elbows'have one (1) 75 inch long seam. Four inches-of this weldLcould not'be're-radiographed due to the sockolet being installed'through the weld. The 71-inches which was.

re-radiographed have coverage to'95%'of the total weld.

. l June 4, 1984 RBG- 17,995 Based on the confirmatory radiography of the accessible welds and the relatively small portion of the weld which was inaccessible, the inaccessible portion is determined to be acceptable.

Corrective Steps Taken To Avoid Further Violations An additional review of other radiographs supplied by General Electric was performed.- As a result of this review, no discrepancies were identified and no additional corrective

. action is required. This is considered to be an isolated case.

Date When Full Compliance Was Achieved

' GSU was in tull compliance as of June 4, 1984.
