'C' leakage tests which failed to meet the criteria of 10 CFR 50 Appendix J.
Sections III.A.5(b),
III.B.3 acceptance and III.C.3, respectively.
These reports are being provided in accordance with 10 CFR, Appendix J.
The Reference transmitted Licensee Event Report 91-020-01 which discussed failure of Essential Service Water system valves EF HV-32 and EF HV-34.
the The failure of the valves caused the Type
'A' test "as found" leakage rate to exceed L.
In the "as left' condition the Integrated Leak Rate Test was considered Successful.
The Reference discussed the actions taken as a result of the valve failures and requested NRC review and approval of the Type
'A' test schedule.
