Pelke of NRC Region III to report that weldolets'of a specified heat number lack l
complete documentation.
Guyon Alloys, Inc. has reported this deficiency in accordance with the requirements of 10CFR21.
This deficiency is potentially reportable under 10CFR50.55(e) and is being tracked as item 131.
j Description of Deficiency Preliminary information indicates that Bonney Forge weldolets lacking chemical analysis documentation have been installed on ASME Class 2 main _ steam safety relief valve (SRV) discharge lines.
The subject fittings were supplied to Guyon Alloys, Inc. as ASME Class 2 fittings by Bonney Forge - Gulf & Western Manufacturing Company.
Subsequently, Guyon Alloys, Inc. was notified by Bonney Forge that the specified material, including the subject fittings, shipped from their Carlinville, Illinois plant was furnished to s
Guyon Alloys, Inc. with incomplete documentation and lacking the chemical overcheck requirement defined by ASME Section III para. NCA-3867.
