In accordance with the directive given in IE Bulletin 83-06, this report identifies'the activities that have been completed to resolve the issue of nonconforming material supplied by the Tube-Line Corporation (T-L).
An investigation has been performed by Bechtel San Francisco Central Supplier Quality (SFHO) Department to determine if any sub-suppliers have supplied Tube-Line material to the Midland Plant. The following is a summary of this investigation and of the corrective actions taken:

To.date, of the 413 letters mailed from SFHO, 85 letters went to vendors that supplied material to the Midland Project. Bechtel Ann Arbor has received 85 (100%) of the Midland responses.

Of the 85 responses, 82 vendors have responded that'no Tube-Line material was furnished to Midland. The three vendors that have furnished Midland Tube-Line material are Guyon Alloys, Liberty Equipment and W J Wooley.

~ )- Nonconformance Report C-00355; identifies 50 fittings received from a
Guyon Alloy and Liberty Equipment, 17 fittings were located in Warehouse No'1 in the separated hold area with Lold tags attached.
- This NCR was dispositioned on May 3, 1984 by indicating that the 17
-fittings were to be returned to the suppliers.

. supplier. This NCR was dispositioned on May 2, 1984.
c)'LNonconformance Report 6022* identifies two 10" caps from W J Wooley that have been incorporated into ASME Class 2 Air Tank OT-921. NCR 6022 has been dispositioned by requiring removal of the caps.
x* Note that-two NCRs 6022 and C-0915 were opened on this subject. NCR C-0915 E
-was voided on May 31, 1984 to eliminate the duplication.
LA' total o'f.52 Tube-Line fittings have been received at the site for use in safety-related systems. Of these', 11 were installed and 17 were located in

37 Lthe warehouse. As stated above, these 28 fittings were identified on nonconformance reports and will;be removed from the site. The remaining 24
' fittings ~are not installed in safety-related systems, based on a review of all pertinent documentation at the jobsite. -The remaining 24 have either been installed in non-safety related systems or have been scrapped.
'The controlled heat log at the site identifies all heats acceptable for w/ '
installation in safety-related systems. All Tube-Line materials have been
< removed from the log..This will ensure that no nonconformance Tube-Line materials'will-be installed in the future.
