ENCLOSURE (1)
REPLY TO APPENDIX A OF NRC INSPECTION REPORT 50-317/84-11; 50-318/84-11
-We have reviewed the circumstances that led to the apparent violation of 10 CFR 50, Appendix B, Criterion XVI requiring timely corrective action to assure that conditions
- such as improper valve labeling of the Hydrogen Analyzers are properly identified and corrected.
The root cause leading to t'ils event was a failure to fully implement corrective action in a manner that would have prevented an apparent violation of valve mislabeling in the Gas Analyzing System. Accordingly, the corrective actions cited below have been or will be implemented to ensure similar violations will not occur in the future.
(1)
A complete review of the Oxygen and Hydrogen Analyzing Cabinets has been performed and all valve labeling discrepancies identified. The discrepancies will be corrected through the on-going efforts of the Valve Labeling Program by September 1,1984.
(2)
This incident was reviewed and discussed with Electrical & Controls Technicians to emphasize the impact of improper equipment labeling and the potential problems that can result. This action was completed on July 17,1984.
