Dear Dr. Murley:

Millstone Nuclear Power Station, Unit No. 3 Reporting of Potential Significant Deficiencies in Design and Construction:

Isolator Cabinets, Main Ventilation, Air Conditioning and Other Miscellaneous Panels with Improperly Crimped Lugs (SD-53)

In a February 15, 1984 telephone conversation between your Mr. T. Rebelowski and our Mr. R. E. Lefebvre, Northeast Nuclear Energy Company (NNECO) reported a potential significant deficiency in the construction of Millstone Unit No. 3 as required by.10CFR50.55(e).

The potential significant deficiency involves inadequate lug crimping in isolator cabinets, main ventilation, air conditioning, and other miscellaneous panels supplied by Systems Control.

Our architect-engineer, Stone & Webster Engineering Corporation, has completed the repair of all improperly crimped lugs in the subject pre-turned-over panels with the exception of the installation of approximately 130 resistors.

These resistors are on order with an expected delivery date of October 1,1984.

Installation of these resistors is expected to be completed by the end of October, 1984.

NNECO has also completed the repair of improperly crimped lugs in all of the post-turned-over panels. However, there exists several turned-over energized circuits within pre-turned-over panels that require replacement of improperly crimped lugs.

These have not yet been replaced, since a power outage is required to facilitate completion. A date for this required outage has not been set pending receipt of materials for other outage work required.
