Docket No. 50-289 Once Through Steam Generator (OTSG) Tubes - Post inservice inspection (ISI) Report for Cycle 11 Refueling (11R) Outage This letter fulfills the TMI-1 Technical Specification 4.19.5.a requirement that GPU Nuclear submit a report to the NRC of the number of tubes repaired or removed from service in each OTSG within 15 days of the completion of each ISI of the OTSG tubes.
During the 11R Outage, our evaluation of the OTSG eddy current inspection results determined the need to remove six defective tubes from service by plugging.
Specifically, one sleeved tube was plugged in OTSG "A" and five tubes were plugged in OTSG "B." The installation of these plugs was completed on September 30,1995.
A total of 1269 tubes have been removed from service in OTSG "A" and 378 in OTSG "B since their fabrication.
