Non-confonnance Response In response to the inspection team's deficiency regarding the dissemination of deviations brought to the attention of Limitorque as a result of a Part 21 filed by a utility, Limitorque will take the following actions. The Part 21 Committee meeting was conducted on June 6,1991 during which, the committee agreed to include the appropriate notification to the industry via a maintenance bulletin letter at its next issuance. In addition, to prevent recurrence QCP-22 has been revised and the following paragraph added to section 22.E. If the committee has information that a licensee has reponed the condition as noted above and in the committee's evaluation it is possible that the deviation may be present at that facility, then the committee shall assure that the issue is addressed in the next maintenance bulletin or advisory mailing to the industry. In addition, a training session has been conducted and documented with the individuals in Nuclear Support and Service discussing the QCP-22 Revision 6 requirement to assure that all notifications from nuclear utility customers regarding failures be reponed to Limitorque in writing. In addition, a discussion was held regarding the responsibilities of all individuals involved with respect to Part 21. Dear IIolland:
Attached please find a copy of the Part 21 issued by Vermont Yankee concerning motor tie bolt failurc. In addition, I have attached a copy of a letter from Yankee Atomic dated November 10,1989 in which Limitorque agreed that the failure mechanism for the tie bolts was as a result ofloss of pretentioning. This issue is being brought to jour attention in order that the necessary evaluations can be addressed by your organization.
